Sternal fracture with fatal outcome in cystic fibrosis.

چکیده
A 16 year old girl with cystic fibrosis (CF), with very severe pulmonary disease and osteoporosis, presented to the outpatient clinic with sudden onset of back pain. A radiograph of the spine revealed multiple vertebral fractures leading to new thoracic kyphosis. She was briefly admitted to hospital for pain control and discharged without respiratory distress. Ten days later she presented again with increasing dyspnoea and difficulties in inflating her chest secondary to a new funnel chest. Despite ongoing respiratory support, appropriate antibiotics, pain control and physiotherapy, the patient died in respiratory failure 2 days after admission. It is likely that the recent vertebral fractures (fig 1, small arrows) generated forces on the sternum that led to the spontaneous fracture of the mid sternum (fig 1, large arrow). Low bone mineral density (BMD) is common in adults and adolescents with CF. Spontaneous fractures of the vertebra and ribs may occur in these patients, especially if they have additional risk factors for the development of bone disease. For example, Elkin et al found evidence of vertebral deformity by radiography in 17% of 107 adult CF patients. Although several well designed studies on bisphosphonates as treatment of CF related bone disease exist, none of these trials included children. This case emphasises the importance of screening for BMD and the need for more aggressive treatment, not only in adults but also in children with CF.

Outcome of Cystic Fibrosis in Patients with Bronchiectasis
Introduction: Bronchiectasis is a common problem in children especially under 5 years. Early diagnosis of disease and its causes could be useful in early treatment and preventing probable complications. This study aimed at evaluating the cystic fibrosis (CF) in patients with bronchiectasis. Methods: In a cross-sectional study, 374 children with bronchiectasis were studied. The diagnosis was ma...
متن کاملPositive Expiratory Pressure (PEP) versus Conventional Chest Physiotherapy in Pediatric Patients with Acute Exacerbation of Cystic Fibrosis
Background Pulmonary involvement is the main cause of mortality in cystic fibrosis (CF). Airway clearance techniques are non-pharmacological complement options for CF patients. The aim of this study was to evaluate the short-term outcome of airway cleaning treatment in patients with cystic fibrosis in a children's hospital. Materials and Methods This clinical trial study conducted on 40 CF pati...
متن کاملSevere nephropathy in three adolescents with cystic fibrosis.
Renal involvement has rarely been reported in patients with cystic fibrosis. We describe severe nephropathy with a rapidly fatal outcome in three adolescents with cystic fibrosis, and evaluate the important repercussions that the nephrotic syndrome had on the precarious clinical situation of these patients.

Participants were a convenience sample of 23 patients with CF, ages 5 to 12 years and their parent(s) in an ambulatory CF clinic. The design was a focused ethnography including in-depth interviews with children and parent(s), children’s drawings and self-administered written parental questionna...
متن کاملInvasive Scedosporium sternal osteomyelitis following lung transplant: Cured
Scedosporium is an important pathogen in cystic fibrosis (CF) and post-transplant but rarely causes invasive infection. Treatment remains challenging, particularly due to inherent resistance to multiple antifungal agents. We present a young man with CF who developed invasive sternal and rib infection 10-months following lung transplant. The infection has been clinically and radiologically cured...
