Teaching NeuroImages: Reversible widespread brain MRI lesions in Marchiafava-Bignami disease.

چکیده
A 54-year-old woman with a history of alcoholism presented with stupor. Brain MRI disclosed extensive callosal and widespread extracallosal white matter and cortical lesions (figure) suggestive of MarchiafavaBignami disease (MBD). After 4 weeks of treatment with thiamine and corticosteroids, a complete resolution of brain MRI lesions along with a good clinical recovery was observed. In MBD, which is rare and frequently associated with alcoholism, brain MRI may show callosal, but also extracallosal, white matter and cortical lesions. In alcoholic MBD patients with impaired consciousness, rapid resolution of the widespread callosal, extracallosal, and cortical MRI lesions is highly unusual.

Regional cerebral glucose metabolism and blood flow in a patient with Marchiafava-Bignami disease.
We report functional neuroimaging studies of a 54-year-old man with Marchiafava-Bignami disease (MBD). Glucose metabolic images obtained by [18F]-2-fluoro-2-deoxy-D-glucose positron emission tomography showed diffusely reduced whole brain metabolism and strongly decreased metabolism in the frontal and parietal lobes, orbital gyrus, and thalamus. Cerebral perfusion images showed a similarly decr...
متن کاملAcute Marchiafava-Bignami disease: clinical and serial MRI correlation.
Marchiafava-Bignami disease (MBD) is a form of toxic demyelinating disease more often seen in chronic alcoholics. The disease process typically involves the corpus callosum and clinically often presents with altered sensorium, neurocognitive defects or seizures with acute cases often deteriorating to comatose state. The death rate is high. We report a rare case of MBD with complete clinical rec...
متن کاملAcute Marchiafava-Bignami Disease with Widespread Callosal and Cortical Lesions
Marchiafava-Bignami disease (MBD) is a rare alcohol-related disorder that results in progressive demyelination and necrosis of the corpus callosum. The process may extend to the optic chiasm and tracts, cerebellar peduncle, subcortical resion, neighboring white matter, and rarely, cortical gray matter. We report a case of MBD in which fluid-attenuated inversion recovery and diffusion magnetic r...
متن کاملAcute Marchiafava-Bignami Disease: Diffusion-Weighted MRI in Cortical and Callosal Involvement
Marchiafava-Bignami disease (MBD) is a fatal disorder characterized by demyelination of the corpus callosum. MRI, suggestive of corpus callosum demyelination with associated white matter involvement in both cerebral hemispheres, indicates a diagnosis of MBD. In this case, MR diffusion-weighted findings taken at an acute stage of MBD revealed lesions not only in the corpus callosum but also in t...
متن کاملThe Importance of Brain MRI in the Diagnosis of Marchiafava-Bignami Disease
Marchiafava-Bignami disease (MBD) is a neurological disorder that has been found to be associated with chronic alcoholism and malnutrition. We report a 45 year old man, chronic alcoholic that developed discouragement for activities involving daily living, changes in retrograde memory in addition to mutism and gait instability. Brain MRI showed central atrophy of the corpus callosum(CC), hypoint...
