The Ross operation.

چکیده
Both congenital and acquired aortic valve defects represent an important group of diseases in pediatric patients. Since the defects –in form of stenosis, insufficiency or mixedmay finally cause a loss of ventricular function and mean a big load for the heart muscle, an interventional or surgical therapy is required. Although there are various alternatives of treatment, none of these is really satisfactory. The balloon valvuloplasty which is especially performed on patients with aortic stenosis does not seem to be a lasting solution. Also the replacement of homografts and mechanical valves in aortic position has not been as successful. Particularly problems like several re-operations, thromboembolic complications, frequent endocarditis, the need of anticoagulants and their risk of bleeding have forced us to search for new ways of therapy. In this respect the Ross procedure is the best option for pediatric patients.

The modified Ross operation using a Dacron prosthetic vascular jacket does prevent pulmonary autograft dilatation at 4.5-year follow-up.
OBJECTIVE Following the Ross operation, pulmonary autografts tend to dilate over time. This study researches the fate of the pulmonary autograft - at 4.5 years following the modified Ross operation - with special reference to the impact of the modification on (a) pulmonary autograft dilatation, (b) the neo-aortic root geometry, (c) neo-aortic valve function and (d) the coronary artery reserve. ...
متن کاملRoss operation for bicuspid aortic valve: the myth, the reality.
I read with great interest the article by Hraska et al. [1], in the May issue of EJCTS, summarizing their 7 years experience with the Ross and Ross–Konno operation in children and adolescents in two centers. Like most other series, their mid-term experience validates the safety and efficacy of Ross operation in treating aortic valve disease in this important subset of patients where growth, act...

In this issue, Andreas et al. [1] and da Costa et al. [2] present their centre's experience with the Ross procedure. In both publications and up to 18 years postoperatively, patient survival after the Ross operation was comparable with that of the general population and was superior compared with that after mechanical valve replacement in a real-world setting [1]. This normal survival is unques...
متن کاملThe Ross Procedure in Pediatric Patients: A 20-Year Experience of Ross Procedure in a Single Institution
BACKGROUND The Ross/Ross-Konno procedure is considered a good option for irreparable aortic valve disease in pediatric patients because of its hemodynamic performance and potential for growth of the pulmonary autograft. This study is a review of the long-term results of our 20-year experience with the Ross and Ross-Konno operations in a single institution. METHODS Between June 1995 and Januar...
متن کاملRoss operation and right ventricular outflow tract reconstruction with stentless xenografts.
Traditionally a homograft valve is used as a pulmonary replacement device for the Ross operation. Right ventricular outflow tract reconstruction during aortic valve replacement with an autograft was performed with stentless xenograft valves in nine patients. Hemodynamic performance is satisfying, but, long term evaluation is needed.
متن کاملThe Ross procedure in children: preoperative haemodynamic manifestation has significant effect on late autograft re-operation.
OBJECTIVES The Ross procedure is the aortic valve-replacement procedure of choice in children. Nonetheless, late autograft re-operation for dilatation and/or valve regurgitation is of concern. We examined whether preoperative haemodynamic manifestation (e.g., stenosis, regurgitation and mixed aortic valve disease) affected late re-operation risk. METHODS Medical records of 227 children who un...
