Erythropoietin Friend or Foe in Chronic Kidney Disease Anemia: An Analysis of Randomized Controlled Trials, Observational Studies and Meta-analyses
نویسنده
چکیده
Background: Anemia is a common clinical problem in patients with chronic kidney disease and is associated with increased morbidity and mortality. Erythropoietin is a hormone synthesized in kidney responsible for red blood cell maturation in the bone marrow. It is deficient in majority of patients with advanced kidney disease predisposing to anemia. Therapeutic alternatives are recombinant human erythropoietin (epoietin alfa), darbepoetin, other similar agents all now collectively known as ‘erythropoiesis stimulating agents and red blood cell transfusions. Objective: The aim of this review is to assess that whether Erythropoietin (EPO) treatment is beneficial or harmful in the management of anemia associated with CKD. Methods: This review is based on randomized controlled trials, observational studies and meta-analyses published between 1989 and 2008 and focusing EPO use in anemia of CKD. Articles were searched in MEDLINE, PubMed, and other electronic databases as well as in online journals.

Effect of pentoxifylline on anemia in patients with chronic kidney disease: an updated systematic review and meta-analysis
Background: Pentoxifylline, a valuable medication with promising clinical characteristics and considerable profile of safety is used in many conditions namely chronic kidney diseases (CKD). However, the decision to prescribe pentoxifylline for anemia in CKD should be based on evidence accrued from randomized controlled trials (RCTs). Yet, substantial heterogeneity exists in studies performed to...
متن کاملThe safety and effectiveness of the current treatment regimen with or without roflumilast in advanced COPD patients: A systematic review and meta-analysis of randomized controlled trials
Background: Chronic obstructive pulmonary disease (COPD) is a chronic respiratory disease, which reduces the lung function and causes respiratory symptoms over time, and it is primarily associated with shortness of breath, cough and sputum production. Roflumilast, which is a long-acting selective inhibitor, reduces the anti-inflammatory effect of the main symptoms of COPD. The aim of ...
متن کاملHemoglobin targets for the anemia of chronic kidney disease: a meta-analysis of randomized, controlled trials.
Anemia affects almost all patients with chronic kidney disease (CKD), reduces quality of life, and is a risk factor for early death. Higher hemoglobin (Hb) targets have been widely advocated because of data from observational studies showing that higher Hb is associated with improved survival and quality of life, but higher Hb targets may cause access thrombosis and hypertension and are costly....
متن کاملAnemia, renal transplantation, and the anemia paradox.
Anemia is prevalent in renal transplant recipients (RTRs), as it is in all chronic kidney disease (CKD) populations. Mild anemia occurs in up to 40% of RTRs, and more severe anemia (110 g/L) occurs in about 9% to 22% of patients. As in CKD, impaired graft (renal) function is a major predictor of anemia identified in nearly all studies, suggesting a major role for erythropoietin deficiency. Chro...

Background: Chronic obstructive pulmonary disease (COPD) is a major health dilemma and cause of morbidity and mortality in either industrialized or developing countries and inhaled corticosteroids are widely used worldwide in these patients. In this systematic review, we aimed to analyze the effectiveness of these gents compared to placebo. Methods: Pubmed and Google Scholar literature search ...
