Should all Hypertensive Patients be Screened for Primary Aldosteronism?
نویسنده
چکیده
Primary Aldosteronism (PA) is defined as non-suppressible hyper secretion of aldosterone by adrenal glands, with consequent suppression of plasma renin activity. It was described by Jerome Conn in 1955 [1]. It is said to be an under diagnosed and important secondary endocrine cause of hypertension, and is one of the few potentially curable causes of hypertension using both medical and surgical interventions [2]. The most common subtypes of PA are bilateral adrenal hyperplasia (60%) and aldosterone-producing adenoma (30%). Other rarer causes include adrenal carcinoma (rare) and glucocorticoid suppressible hyperaldosteronism (rare).

Would wider screening for primary aldosteronism give any health benefits?
Fifty years ago, Jerome Conn described 'a new clinical syndrome which is designated temporarily as primary aldosteronism' in a young patient with hypertension, severe hypokalaemia and a benign adrenocortical tumour, for whom an adrenalectomy cured both the hypertension and hypokalaemia. His report identified the condition known as aldosterone-producing adenoma, a form of curable hypertension. A...
متن کاملPeriodic Paralysis: An Uncommon Complication of Primary Aldosteronism
Periodic paralysis is an uncommon complication of primary aldosteronism in the non-Asian population. We describe the case of a Brazilian woman who presented to the emergency room with proximal symmetric tetraparesis that was later diagnosed as primary aldosteronism. This case report shows that primary aldosteronism should be included in the differential diagnosis of periodic paralysis, especial...
متن کاملMalignant hypertension and hypertensive encephalopathy in primary aldosteronism caused by adrenal adenoma.
Two cases are reported as follows: 1) 1 female patient with accelerated-malignant hypertension secondary to an aldosterone-producing adrenal adenoma; and 2) 1 female patient with adrenal adenoma, severe hypertension, and hypertensive encephalopathy. This association is a rare clinical finding, and malignant hypertension may modify the hormonal characteristic of primary aldosteronism, making its...
متن کاملPrimary aldosteronism in diabetic subjects with resistant hypertension: response to Umpierrez et al.
OBJECTIVE Despite the high prevalence of hypertension in patients with type 2 diabetes, the prevalence of primary aldosteronism in this population has not been determined. RESEARCH DESIGN AND METHODS One hundred subjects with type 2 diabetes and resistant hypertension, defined as blood pressure > 140/90 mmHg despite the use of > or = 3 antihypertensive agents, were screened for primary aldost...
متن کاملMinireview: primary aldosteronism--changing concepts in diagnosis and treatment.
Primary aldosteronism affects 5-13% of patients with hypertension. Patients with hypertension and hypokalemia and most patients with treatment-resistant hypertension should undergo screening for primary aldosteronism with a plasma aldosterone concentration to plasma renin activity ratio. A high plasma aldosterone concentration to plasma renin activity ratio is a positive screening test result, ...
