چکیده

Dissociated strabismus complex (DSC), comprising dissociated vertical deviation (DVD), dissociated horizontal deviation (DHD), and dissociated torsional deviation (DTD), is an enigmatic ocular misalignment disorder, for which multiple causes have been hypothesized. Evidence-based studies evaluating the pathophysiology of this unusual ocular motility disorder is of value to existing knowledge. Luminance disparity and fixation loss have been implicated in the pathogenesis of DVD and form the basis of classical clinical tests considered characteristic of this condition, like the Bielschowsky phenomenon. While occlusion with minimal luminance imbalance has been found to result in DVD, the effect of luminance disparity alone as a trigger for DVD has not been assessed. This ageand sex-matched case-control study by Ghadban et al. evaluates the role of isolated binocular luminance disparity in the causation of DVD. The amplitude of vertical deviation in the same set of patients with DVD as a response to marked binocular luminance disparity, uniocular occlusion, and alternate occlusion maintaining similar background illumination (either light or dark) was systemically evaluated with video-oculography (VOG). The observations conclusively emphasized that luminance disparity alone has little effect on the genesis of DVD, suppression of fixational effort being the fundamental primary stimulus. It was postulated that this loss of effort, which may be due to uniocular occlusion or blurring or contralateral fixation, when superimposed with binocular luminance disparity at the cortical level, triggered DVD with demonstration of the characteristic Beilschowsky phenomena. This phenomenon is unique to DVD because the control group demonstrated no such deviation under any of the four test conditions. Overall, the design and execution of the study has been meticulously focused towards delineating the hitherto unanalyzed independent roles of luminance disparity and fixation loss in the pathogenesis and behavior of the vertical deviation in DVD, amalgamated with an excellent scientific review and illustrative recording.

Relative roles of luminance and fixation in inducing dissociated vertical divergence.

PURPOSE We evaluated the roles of luminance and fixation in the pathophysiology of dissociated vertical divergence (DVD). METHODS Vertical eye position was measured in 6 subjects with DVD (ages 11-47 years, 5 females) and 6 controls (ages 16-40 years, 5 females) using video-oculography (VOG) under conditions of change in fixation and luminance. RESULTS Subjects with DVD showed the following...

Functional MRI of the superior colliculus in dissociated vertical deviation.

Dissociated vertical deviation (DVD), first described in 1894, is commonly encountered in the infantile strabismus syndrome. The vertical position of the eyes in DVD depends on fixation effort and luminance differences. The neural mechanism behind DVD is still not clear. Visual and oculomotor pathways in the brainstem are phylogenetically old pathways which in origin are basically monocular: th...

Stability of human binocular alignment in the dark and under conditions of nonfixation.

PURPOSE To evaluate the stability of human binocular alignment under conditions of altered fixation and luminance. METHODS Horizontal binocular alignment in 8 healthy orthotropic subjects was measured using infrared video-oculography (VOG) under conditions of binocular fixation and luminance change. Each testing condition was preceded by a binocular fixation period in room light (475 lux) to ...

Facilitation of vertical vergence by horizontal saccades, found in a patient with dissociated vertical deviation.

The authors examined vertical vergence in a 15-year-old girl with dissociated vertical deviation, a 6d` convergent strabismus, no binocular vision, latent nystagmus, and a minimal left amblyopia. Eye movements were recorded during 4s-periods of (1) both eyes open, alternated with 4s-periods of (2a) right eye covered, (2b) left eye covered or (2c) both eyes closed. The patient preferred fixation...

Alternating fixation and saccade behavior in nonhuman primates with alternating occlusion-induced exotropia.

PURPOSE Nonhuman primates reared with daily alternating monocular occlusion (AMO) during their first few months of life develop large horizontal strabismus, A/V patterns and dissociated vertical deviation (DVD). In addition, these animals often alternate or switch the fixating eye during binocular viewing. The purpose of this study was to characterize the alternating fixation behavior of these ...
