Diagnostic techniques for ventilator-associated pneumonia: Conflicting results from two trials
نویسندگان
چکیده
Intervention: Using a 2-by-2 factorial design, subjects were randomly assigned to a) undergo bronchoalveolar lavage with quantitative culture of the bronchoalveolar-lavage fluid or endotracheal aspiration with nonquantitative culture of the aspirate, and to b) receive empirical combination antibiotic therapy or monotherapy. Empirical antibiotic therapy was initiated in all patients until culture results were available, at which point a protocol of targeted therapy was used for discontinuing or reducing the dose or number of antibiotics, or for resuming antibiotic therapy to treat a preenrollment condition if the culture was negative.

A randomized trial of diagnostic techniques for ventilator-associated pneumonia.
BACKGROUND Critically ill patients who require mechanical ventilation are at risk for ventilator-associated pneumonia. Current data are conflicting as to the optimal diagnostic approach in patients who have suspected ventilator-associated pneumonia. METHODS In a multicenter trial, we randomly assigned immunocompetent adults who were receiving mechanical ventilation and who had suspected venti...
متن کاملComparing the Effect of Using Normal Saline and Distilled Water for Tracheal Suctioning on the Incidence of Ventilator-associated Pneumonia and Hemodynamic Indexes in Ventilator-dependent Patients
Background and Aims: VAP prevention is a top priority in the intensive care units. The study was conducted to compare the effect of using normal saline and distilled water for tracheal suctioning on the incidence of ventilator-associated pneumonia and hemodynamic indexes in ventilator-dependent patients. Methods: The present study is a randomized clinical trial that was conducted from 2017 to 2...
متن کاملInvasive approaches to the diagnosis of ventilator-associated pneumonia: a meta-analysis.
OBJECTIVE Ventilator-associated pneumonia remains a major challenge in the intensive care unit. The role for invasive diagnostic methods (e.g., bronchoscopy) remains unclear. We hypothesized that invasive testing would alter antibiotic management in patients with ventilator-associated pneumonia but would not necessarily alter mortality. DESIGN Meta-analysis of randomized, controlled trials of...
متن کاملEvaluation of the Effects of Oronasal Versus Oral Disinfections with Chlorhexidine on Clinical Criteria of Ventilator-associated Pneumonia
Background: Ventilator-associated pneumonia (VAP) is the most prevalent and lethal form of nosocomial infections in the ICU and oral disinfection is a nursing measure to prevent this condition. Aim: this study aimed to evaluate the effect of oronasal versus oral disinfections with chlorhexidine on the clinical criteria for diagnosis of VAP. Method: This randomized clinical trial was conducted o...
متن کاملعنوان: Impact of Care Program on Ventilator-Associated Pneumonia Incidence: A Clinical Trial
چکیده: Background: Ventilator-associated pneumonia (VAP) is one of the common problems in patients who are under mechanical ventilation and increases the hospitalization duration and treatment expenses. The current study was conducted to determine the impact of a care program on VAP. Methods: This clinical trial was performed on intubated patients who were under mechanical ventilation in Intens...
