The control of tonic pain by active 1 relief learning

چکیده
15 Tonic pain after injury characterises a behavioural state that prioritises recovery. Although generally 16 suppressing cognition and attention, tonic pain needs to allow effective relief learning to reduce 17 the cause of the pain. Here, we describe a central learning circuit that supports learning of relief 18 and concurrently suppresses the level of ongoing pain. We used computational modelling of 19 behavioural, physiological and neuroimaging data in two experiments in which subjects learned to 20 terminate tonic pain in static and dynamic escape-learning paradigms. In both studies, we show 21 that active relief-seeking involves a reinforcement learning process manifest by error signals 22 observed in the dorsal putamen. Critically, this system uses an uncertainty (‘associability’) signal 23 detected in pregenual anterior cingulate cortex that both controls the relief learning rate, and 24 endogenously and parametrically modulates the level of tonic pain. The results define a 25 self-organising learning circuit that reduces ongoing pain when learning about potential relief. 26

متن کاملImproved Pain Relief With Burst Spinal Cord Stimulation for Two Weeks in Patients Using Tonic Stimulation: Results From a Small Clinical Study
OBJECTIVES Conventional spinal cord stimulation (SCS) delivers a tonic waveform with consistent stream of pulses; burst delivers groups of pulses separated by short pulse-free periods. The current study compared the short-term safety and efficacy of burst with tonic stimulation in subjects already receiving SCS. MATERIALS AND METHODS At 4 IRB-approved sites, 22 subjects previously implanted w...
متن کاملClinical Compendium BURST STIMULATION TECHNOLOGY A Novel Stimulation Mode for the Management of Chronic Intractable Pain
St. Jude Medical has the first and only CE-marked neurostimulation system that delivers both Burst and tonic spinal cord stimulation (SCS). 1 Burst stimulation technology has been shown to further reduce pain in tonic responders with a reduction in paresthesia and recapture pain relief in non-responders to traditional tonic stimulation. When used to manage chronic, intractable pain of the trunk...

Aims and Background: Pain is an inevitable part of the childbirth process. Nowadays non-pharmacological methods such as acupuncture are considered for labor pain relief, due to their lower side effects. So this study was conducted to evaluate the effect of Auriculotherapy on pain intensity during the active phase of labor. Materials and Methods: This was a clinical trial on 80 nulliparous-women...
متن کاملThe effects of lesions of the nucleus reticularis paragigantocellularis on tonic pain
In the present study, the effect of contralateral and bilateral electrolytic lesions of the nucleus reticularis paragigantocellularis (PGi) on tonic pain was assessed in rats. Pain-related behavior was evaluated using the formalin test 7 days after the lesion was made. PGi lesions did not produce any marked changes in the early phase of the formalin test, but significantly increased pain sens...
