چکیده

Current therapies for asthma are aimed at controlling disease symptoms and for the majority of patients inhaled glucocorticoid anti-inflammatory therapy is both effective and well-tolerated. However, concerns remain about the adverse effects of glucocorticoids while a subset of asthmatic patients remains symptomatic despite optimal treatment thereby creating a clear unmet medical need. There is considerable evidence that implicates eosinophils as important effector cells and immunomodulators in the inflammation characteristic of asthma. Numerous in vitro and animal studies have demonstrated essential roles for cell adhesion molecules in eosinophil adhesion and transendothelial migration including the selectins, ICAM-1, VCAM-1 together with many of the 1 and 2 integrins. A large body of evidence has also implicated several cytokines and chemokines in the selective recruitment of eosinophils to sites of asthmatic inflammation. Biopharmaceutical approaches have been used to identify inhibitory molecules that target key elements in the processes controlling eosinophil accumulation in asthma. This review will summarise the problems and successes regarding recent developments in therapeutic strategies aimed at reducing eosinophil-mediated inflammation in the asthmatic

Targeting interleukin (IL) 5 for asthma and hypereosinophilic diseases.

Although glucocorticosteroids are still the first line of treatment for chronic asthma, over the last two decades great advances have been made in understanding the pathogenesis of asthma that enabled the identification of new therapeutic targets for asthma treatment. The interleukin (IL) 5: eosinophil axis is a hallmark pathway of allergic inflammation that has received much attention. Indeed,...

The IL-3/IL-5/GM-CSF common receptor plays a pivotal role in the regulation of Th2 immunity and allergic airway inflammation.

The eosinophil is a central effector cell in allergic asthma. Differentiation and function of eosinophils are regulated by the CD4 Th2 cytokines IL-3, IL-5, and GM-CSF, which all signal through a common beta receptor subunit (betac). Recent therapeutic approaches targeting IL-5 alone have not ablated tissue accumulation of eosinophils and have had limited effects on disease progression, suggest...

Therapeutic Targeting of Eosinophil Adhesion and Accumulation in Allergic Conjunctivitis

Considerable evidence indicates that eosinophils are important effectors of ocular allergy. Increased worldwide prevalence of allergic eye pathologies has stimulated the identification of novel drug targets, including eosinophils and adhesion molecules. Accumulation of eosinophils in the eye is a key event in the onset and maintenance of allergic inflammation and is mediated by different adhesi...

Expression of the Apoptosis Inhibitor Bcl-2 in Sputum Eosinophils in Patients with Asthma

Background and Objectives: Apoptosis of eosinophils is of significant value in assessing the airway inflammation in patients with asthma. Our purpose was to investigate the degree of expression of the Bcl-2 protein in sputum eosinophils during acute asthma exacerbation and its relationship with exacerbation severity.    Materials and Methods: The study was carried out in Mostafa Khomeini Ho...

Therapeutic interventions in severe asthma

The present paper addresses severe asthma which is limited to 5-10% of the overall population of asthmatics. However, it accounts for 50% or more of socials costs of the disease, as it is responsible for hospitalizations and Emergency Department accesses as well as expensive treatments. The recent identification of different endotypes of asthma, based on the inflammatory pattern, has led to the...
