One-stage Pantalar Arthrodesis for Ankle-foot Deformity in Post-poliomyelitis Patients
نویسنده
چکیده
Post-poliomyelitis foot deformity remains a challenging problem to orthopedic surgeons, though its incidence has decreased greatly. The purpose of this retrospective study was to evaluate the surgical outcome in patients with paralytic foot deformity associated with poliomyelitis. Between 1998 and 2002, seven patients with a history of prolonged, severe, and rigid equinocavovarus deformity secondary to poliomyelitis underwent one-stage deformity correction and pantalar arthrodesis. Their average age at the time of surgery was 54 years and the average follow-up was 22 months. Fixation with multiple cannulated screws, bone staples, or K-wires was used in all patients. After surgery, no vascular insufficiency or skin sloughing developed. At the latest follow-up, solid arthrodesis was achieved in all patients and none of the deformities recurred. Painless plantigrade feet were achieved in all patients but one. Leg length discrepancy averaged 2.7 cm (range, 24.5 cm), which was managed with shoe lifts. In conclusion, with stringent preoperative evaluation, meticulous surgical technique, and highly compliant postoperative cast protection, one-stage deformity correction and pantalar arthrodesis can be successfully accomplished in patients with severe, rigid equinocavovarus deformity secondary to poliomyelitis.

Background: There is still controversy regarding the best technique for ankle arthrodesis to acheive stable rigid fixation along with reconstructing a functional plantigrade foot. Moreover, existing techniques have complictions related to stability, soft tissue covering, fusion rate, and exposure. Methods: With the anterior approach exactly on the tibialis anterior sheath, the joint was expose...

Seventy-five years have passed since subtalar arthrodesis was first performed, by Albert of Vienna (1 882), to counteract lateral instability in the paralytic club foot, but to Davis (1892) is due the main credit for developing the operation as an established procedure. Soon afterwards Whitman (1901) devised “ astragalectomy “ [excision of the talus] for calcaneus deformities, an operation whic...

BACKGROUND There is a scarcity of studies concerning the treatment of foot and talocrural deformities secondary to a compartment syndrome or ischaemia. MATERIAL AND METHODS Five patients (4 males and 1 female) were treated by talocrural arthrodesis (1 foot) and triple tarsal arthrodesis (4 feet) due to equinovarus foot deformity secondary to compartment syndrome or ischaemia. The age at surge...
متن کاملRole of Surgery in Management of Osteo-Articular Tuberculosis of the Foot and Ankle
BACKGROUND Tuberculosis of the foot and ankle still remains to be a significant problem, especially in the developing countries, and with an increase in incidence in immunosuppressed patients. Treatment is mainly medical using multidrug chemotherapy; surgical interventions range from biopsy, synovectomy and debridement, to joint preserving procedures like distraction in early cases, and arthrod...
