Predictive Modeling in Action: How ‘Virtual Wards’ Help High-Risk Patients Receive Hospital Care at Home

چکیده

A model of home-based coordinated care deployed recently in the United Kingdom offers promise in reducing hospital admissions in a relatively low-cost manner. The “virtual ward” program provides multidisciplinary case management services to people who have been identified, using a predictive model, as high risks for future emergency hospitalization. Virtual wards use the systems, staffing, and daily routine of a hospital ward to deliver preventive care to these patients in their own homes. Patients report that they value the improved coordination of their care, while staff report satisfaction in working on a virtual ward and the opportunity to share problems and find solutions with colleagues who are caring for the same patients. With its intuitive appeal, the virtual ward model has been deployed in a number of locations in the U.K. and internationally, including the United States.

Methods for predicting risk of emergency hospitalisation: promoting self-care and integrated service responses in the home to the most vulnerable

Purpose: This projectydevelopment paper outlines the development of an algorithm-based predictive model in England (the King’s Fund’s PARR tool) that uses routine data to identify people at risk of re-admission to hospital. It examines how the model, when linked with data collected from general practices, can and has been used as a method for organizing integrated care services in the home envi...

Do ‘virtual wards’ reduce rates of unplanned hospital admissions, and at what cost? A research protocol using propensity matched controls

BACKGROUND This retrospective study will assess the extent to which multidisciplinary case management in the form of virtual wards (VWs) leads to changes in the use of health care and social care by patients at high risk of future unplanned hospital admission. VWs use the staffing, systems and daily routines of a hospital ward to deliver coordinated care to patients in their own homes. Admissio...

Effect of a postdischarge virtual ward on readmission or death for high-risk patients: a randomized clinical trial.

IMPORTANCE Hospital readmissions are common and costly, and no single intervention or bundle of interventions has reliably reduced readmissions. Virtual wards, which use elements of hospital care in the community, have the potential to reduce readmissions, but have not yet been rigorously evaluated. OBJECTIVE To determine whether a virtual ward-a model of care that uses some of the systems of...

Geographical variation analysis of all-cause hospital readmission cases in Winnipeg, Canada

BACKGROUND Hospital readmission is costly and potentially avoidable. The concept of virtual wards as a new model of care is intended to reduce hospital readmissions by providing short-term transitional care to high-risk and complex patients in the community.
