چکیده

C OMPLEX ALTERATION of blood coagulation occurs in the course of a number of systemic disorders. In contrast to the congenital defects of hemnostasis which are usually characterized by a single clotting factor deficiency, the changes which may accompany acquired disease are characteristicallv multiple. These multiple defects tend to form typical patterns which reflect the physiologic abnormality responsible for them. Accordingly,

Current concepts of hemostasis: implications for therapy.

The revised model of coagulation has implications for therapy of both hemorrhagic and thrombotic disorders. Of particular interest to anesthesiologists is the management of clotting abnormalities before, during, and after surgery. Most hereditary and acquired coagulation factor deficiencies can be managed by specific replacement therapy using clotting factor concentrates. Specific guidelines ha...

Factor VII Gene Defects: Review of Functional Studies and Their Clinical Implications

Coagulation factors belong to a family of plasma glycosylated proteins that should be activated for appropriate blood coagulation. Congenital deficiencies of these factors cause inheritable hemorrhagic diseases. Factor VII (FVII) deficiency is a rare bleeding disorder with variable clinical symptoms. Various mutations have been identified throughout the F7 gene and can affect all the protein do...

Blood component support in acquired coagulopathic conditions: is there a method to the madness?

Acquired coagulopathies are often detected by laboratory investigation in clinical practice. There is a poor correlation between mild to moderate abnormalities of laboratory test and bleeding tendency. Patients who are bleeding due to coagulopathy are often managed with various blood components including plasma, platelets, and cryoprecipitate. However, prophylactic transfusion of these products...

Prevalence and significance of coagulation abnormalities in community-acquired pneumonia.

Coagulation abnormalities are common in severe pneumonia and sepsis, yet little is known about the presence of coagulopathy or its significance in patients with lesser illness severity. We examined coagulation abnormalities in 939 subjects hospitalized with community-acquired pneumonia (CAP) in 28 US hospitals, hypothesizing that abnormalities would increase with illness severity and poor outco...

Pre-anaesthetic assessment of coagulation abnormalities in obstetric patients: usefulness, timing and clinical implications.

The usefulness and optimal timing of laboratory coagulation tests before obstetric extradural analgesia are controversial. Moreover, the significance of mild coagulation abnormalities during pregnancy remains unclear. We have assessed the reliability of coagulation tests performed several weeks before delivery as predictors of coagulation abnormalities during labour. Platelet count, plasma fibr...
