Authors’ contributions All authors made substantial contributions to conception and design and have been involved in drafting the manuscript or revising it critically for important intellectual content. EC conducted the literature review, convened and facilitated the researcher workshop, conducted the focus groups and analysed the data. GH conducted and analysed the qualitative evaluation interviews with carers and carer liaisons. AL conducted, analysed and reported the qualitative evaluation interviews with general practitioners. EC and AJ drafted the manuscript. All authors read and approved the final manuscript.
