چکیده

Methods Controlled, prospective, monocentric observational study. We included 30 mechanically ventilated, critically ill patients (SOFA score ≥ 8 at 3 within 5 days after ICU admission). Two open muscle biopsies (M. vastus lateralis) were obtained at median day 5 and 16. Electron microscope studies were performed and counting of myosin filaments in cross section was done using Image J Software. Reference values were taken from Riley et al.. Muscle strength was assessed using Medical Research Council (MRC)-Scale when patients were alert later in ICU course. Non-parametric tests were performed. Ethic vote (Charité EA2/061/06).

Early activity in the ICU: beyond safety and feasibility.

Often critical care providers think that critically ill patients are too sick to tolerate vigorous activity in the early phase of their illness, thereby incurring “unavoidable” prolonged immobilization. Prolonged immobilization is a substantial contributor to neuromuscular abnormalities, resulting in weakness that may complicate the clinical course of a majority of critically ill patients.1-5 I...

Intensive care unit—acquired weakness (ICUAW) and muscle wasting in critically ill patients with severe sepsis and septic shock

Sepsis presents a major health care problem and remains one of the leading causes of death within the intensive care unit (ICU). Therapeutic approaches against severe sepsis and septic shock focus on early identification. Adequate source control, administration of antibiotics, preload optimization by fluid resuscitation and further hemodynamic stabilisation using vasopressors whenever appropria...

The complex interplay between delirium, sedation, and early mobility during critical illness: applications in the trauma unit.

PURPOSE OF REVIEW Critically ill patients are prescribed sedatives and analgesics to decrease pain and anxiety, improve patient-ventilator dyssynchrony and ensure patient safety. These medications may themselves lead to delirium and ICU-acquired weakness, which are associated with worse clinical outcomes. This review will focus on the epidemiology of these two disease processes and discuss stra...

First results about recovery of walking function in patients with intensive care unit-acquired muscle weakness from the General Weakness Syndrome Therapy (GymNAST) cohort study

OBJECTIVES To describe the time course of recovery of walking function and other activities of daily living in patients with intensive care unit (ICU)-acquired muscle weakness. DESIGN This is a cohort study. PARTICIPANTS We included critically ill patients with ICU-acquired muscle weakness. SETTING Post-acute ICU and rehabilitation units in Germany. MEASURES We measured walking function...

Engaging Critically Ill Patients in Symptom Management: Thinking Outside the Box!

Caring for critically ill patients receiving mechanical ventilation in the intensive care unit (ICU) is an immense challenge for clinicians. Interventions to maintain physiological stability and life itself can cause a number of adverse effects that have a marked impact on patients beyond the period of critical illness or injury. These ICU-acquired conditions include but are not limited to weak...
