Physostigmine as an Antidote

چکیده
Introduction: Rapid treatment of patients with anticholinergic toxicity in the Ernergcncy Department (ED) has remained problematic for many years. These patients are often agitated. uncooperative, and unable to provide a clear history. Physostigmine is a specific antidote for anticholinergic toxicity. yet its popularity has waned due to controversy surrounding its potential central nervo~is system and cardiac toxicities. In this brief review, we discuss physostigmine's history, method of action. indications, contraindications, dosing, and its use in the ED.

The first use of physostigmine in the treatment of atropine poisoning. A translation of Kleinwachter's paper entitled 'Observations on the effect of Calabar bean extract as an antidote to atropine poisoning'.
This paper describes a clinical experiment performed by Drs Kleinwächter and Niemetschek in Prague in 1864, which showed for the first time that oral Calabar bean extract (which contains physostigmine) reverses the toxic effects of atropine. Kleinwächter’s original paper is translated from the German. This paper relates the earliest known account of the use of physostigmine (as an extract of Ca...

The acute toxicity of organophosphorus (OP) compounds in mammals is due to their irreversible inhibition of acetylcholinesterase (AChE) in the nervous system, which leads to increased synaptic acetylcholine levels. The protective actions of intravenously (i.v.) administered pyridostigmine, physostigmine, eptastigmine, and an organophosphate hydrolase, phosphotriesterase, in acute sarin intoxica...
متن کامل[Intoxication with henbane].
Henbane, Hyoscyamus niger, is a hallucinogenic plant, widely spread and easy accessible, which contains anticholinergic substances. Ingestion, intentional or accidental, can provoke serious worsening of psychophysical state and can cause even death. Intoxication resembles the one with atropine. Symptoms like mydriasis, tachycardia, arrhythmia, agitation, convulsion and coma can appear. Diagnosi...
متن کاملA rarely described use of neostigmine in a case of acute anticholinergic poisoning.
Introduction Anticholinergic poisoning can require administration of an antidote. The classical antidote used is the anticholinesterase, physostigmine [1]. Another anticholinesterase, neostigmine, has been less described for this indication [2]. Tropatepine (Lepticur) is a synthetic molecule used in the treatment of Parkinson's disease or to correct Parkinson syndromes induced by neuroleptic dr...
متن کاملWhat to do with case reports: is folly that succeeds folly nonetheless?
Clinical toxicology is a rapidly evolving discipline. Each new pharmaceutical agent, illicit drug, industrial chemical, and household product carries the potential for human toxicity. In addition, combinations of these agents offer limitless possibilities for adverse events, all of which need to be interpreted and shared. Case reports provide the perfect forum for the initial discussion of thes...
