چکیده

While the anatomy of the coronary arteries in complete transposition of the great vessels has been studied by several authors, the overall problem of adequacy of the coronary blood supply in this condition has received less attention. Spitzer (1923) described the probable embryological basis for partial or complete transposition of the coronary arteries. Astley and Parsons (1952) thought that myocardial ischmmia was an important factor in causing cardiac-enlargement in cases of complete transposition of the great vessels. Keith et al. (1953) dissected the coronary arteries in 37 cases of complete transposition, and in 1958 reviewed their anatomy in 80 cases, differentiating them into four main patterns. With the development of techniques for the correction of complete transposition of the great vessels such considerations are of considerable importance. Apart from the anatomical abnormalities that may occur, e.g. a single coronary artery, an ischaemic myocardium may be partly responsible for the high mortality from any form of surgical procedure in cases of complete transposition. The object of this paper is to report the cases of complete transposition of the great vessels seen in Guy's Hospital in whom any abnormal feature of the coronary arteries, the myocardium, or the endocardium was found at autopsy. Other reports about this are reviewed, and the probable explanation of myocardial ischwmia in cases of complete transposition is discussed.

A 33-day-old Infant with the Transposition of the Great Arteries; A Rare Case Report

Dextro-transposition of the great arteries (d-TGA) is the one most common cyanotic congenital heart disease in neonates. The discordant ventriculoarterial arrangement results in parallel circulation, it so is vital to understand the management. We report a rare interesting but critical case of 33-day-old boy who developed cyanosis and had transposition of great arteries combined with interrupte...

A new technic for complete correction of transposition of the great vessels. An experimental study with a preliminary clinical report.

COMPLETE TRANSPOSITION of the great vessels is a common congenital cardiovascular anomaly. In this disease the aorta with its coronary arteries originates from the right ventricle, and the pulmonary artery arises from the left ventricle. Consequently , the aorta carries venous blood; the pulmonary artery, oxygenated blood. Surgical correction of this malformation has been beset with many diffic...

بررسی الگوهای شریان کرونری در جمعیتی از نوزادان مبتلا به جابجایی شریان های بزرگ

Background and purpose: Nowadays cardiovascular developments in pumping of heart-lung and improvements in anesthesia care, including intubation, have made early surgical intervention possible to correct life-threatening congenital anatomic abnormalities in patients of lower ages. This study was performed to evaluate the coronary artery patterns in neonates with transposition of the great arteri...

Transposition of the great vessels with hypoplasia of the right ventricle.

Eight cases are reported in which complete transposition of the great vessels was associated with varying degrees of hypoplasia of the right ventricle. In three, the correct diagnosis was suspected on the basis of certain electrovectorcardiographic findings and confirmed by selective angiography. Quantitative analysis of ventricular volumes and valve circumferences demonstrated that these cases...

Cardiac Resynchronization Therapy Device Implantation in a Patient with Congenitally Corrected Transposition of Great Vessels

A 29-year-old woman was referred to our hospital due to exacerbation in dyspnea on exertion and easy fatigability. A known case of congenitally corrected transposition of the great vessels and congenital complete heart block, she had already received a permanent single-chamber pacemaker. Decision was made to implant a biventricular pacemaker for the treatment of the failing heart. Excellent cor...
