Real-time markerless tumor tracking with MV imaging and a DMLC

چکیده

Innovation/Impact: Robust real-time aperture adaptation for lung tumors during radiation therapy promises better local control while more healthy tissue can be spared. In this context we present the first implementation of a real-time adaptive therapy system using soft tissue localization (STiL) on continuously throughout the treatment acquired cine EPID images to maintain a dynamic treatment aperture centered on the current tumor location during the entire breathing cycle. This approach is superior to other proposed tumor localization methods that rely on implanted fiducial markers (pneumothorax risk), additional imaging dose, external surrogates that may be poorly correlated to the tumor location and/or non-optimal imaging perspectives [1].

First demonstration of combined kV/MV image-guided real-time dynamic multileaf-collimator target tracking.

PURPOSE For intrafraction motion management, a real-time tracking system was developed by combining fiducial marker-based tracking via simultaneous kilovoltage (kV) and megavoltage (MV) imaging and a dynamic multileaf collimator (DMLC) beam-tracking system. METHODS AND MATERIALS The integrated tracking system employed a Varian Trilogy system equipped with kV/MV imaging systems and a Millenniu...

Detailed analysis of latencies in image-based dynamic MLC tracking.

PURPOSE Previous measurements of the accuracy of image-based real-time dynamic multileaf collimator (DMLC) tracking show that the major contributor to errors is latency, i.e., the delay between target motion and MLC response. Therefore the purpose of this work was to develop a method for detailed analysis of latency contributions during image-based DMLC tracking. METHODS A prototype DMLC trac...

Physics Contribution Integration of Real-time Internal Electromagnetic Position Monitoring Coupled with Dynamic Multileaf Collimator Tracking: an Intensity-modulated Radiation Therapy Feasibility Study

Purpose: Continuous tumor position measurement coupled with a tumor tracking system would result in a highly accurate radiation therapy system. Previous internal position monitoring systems have been limited by fluoroscopic radiation dose and low delivery efficiency. We aimed to incorporate a continuous, electromagnetic, threedimensional position tracking system (Calypso 4D Localization System)...

Management of three-dimensional intrafraction motion through real-time DMLC tracking.

Tumor tracking using a dynamic multileaf collimator (DMLC) represents a promising approach for intrafraction motion management in thoracic and abdominal cancer radiotherapy. In this work, we develop, empirically demonstrate, and characterize a novel 3D tracking algorithm for real-time, conformal, intensity modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT)-based radiation...

Electromagnetic detection and real-time DMLC adaptation to target rotation during radiotherapy.

PURPOSE Intrafraction rotation of more than 45° and 25° has been observed for lung and prostate tumors, respectively. Such rotation is not routinely adapted to during current radiotherapy, which may compromise tumor dose coverage. The aim of the study was to investigate the geometric and dosimetric performance of an electromagnetically guided real-time dynamic multileaf collimator (DMLC) tracki...
