LECTURE Dynamic carpal stability

چکیده
The term carpal instability is commonly used, but what carpal stability actually is has not been defined. Much of the mechanically complex wrist’s versatility is due to the intercalated three bone proximal carpal row. Landsmeer described the collapse tendency associated with intercalated segments. The factors which provide static stability are the oblique alignment of the scaphoid, the obliquely aligned dorsal and palmar ligamentous complexs, the intrinsic perilunate ligaments, the transiting transcarpal tendons and the negative intraarticular pressure. The proximal carpal row adjusts its posture on the counterbalancing flexion/pronation torque exerted by the scaphoid and the extension/supination torque exerted by the triquetrum. The dynamic factors are the compressive force exerted across the joint acting on the joint surfaces and the effect of the bowstringing force provided by the flexor carpi radialis acting at the scaphoid tuberosity. The proximal carpal row has a tendency to translate ulnarly along the ulnarly sloped radial articular surface while the distal row has a tendency to slide radially on the radially sloped lunatatotriqueteral distal articular surface. This activity produces differential tension in the ligaments attaching to the triquetrum which effects an extension/supination stance of the triquetrum. The force couple acting on the scaphoid effects the flexion tendency of the scaphoid. The bowstringing of the flexor carpi radialis also counteracts scaphoid flexion. Alterations in any of these factors may upset the delicate mechanical balance of the joint. (Keio J Med 51 (3): 140– 147, September 2002)

Bilateral Pancarpal Arthrodesis in a German Shepherd Dog
Case Description- A 5-year old male German shepherd weighting 28 kg with the history of falling from the third floor 9 month previous was referred to the Small Animal Hospital. Clinical Findings- In clinical examination roughness and thickness of the skin of the palmar surface of both forelimbs were felt. The animal was using his palmar plane of both forelimbs to walk without any signs of pain ...

OBJECTIVE To report treatment of a unilateral comminuted fourth carpal bone (C4) fracture associated with carpal instability by partial carpal arthrodesis (PCA) of the middle carpal joint (MCJ) and carpometacarpal joint (CMCJ). STUDY DESIGN Case Report. ANIMALS An 8-month-old Arabian filly. METHODS A C4 slab fracture was diagnosed radiographically; however, fracture comminution was conclu...
متن کاملDynamic high-resolution ultrasound of intrinsic and extrinsic ligaments of the wrist: How to make it simple.
Wrist ligaments are crucial structures for the maintenance of carpal stability. They are classified into extrinsic ligaments, connecting the carpus with the forearm bones or distal radioulnar ligaments, and intrinsic ligaments, entirely situated within the carpus. Lesions of intrinsic and extrinsic ligaments of the wrist have been demonstrated to occur largely, mostly in patients with history o...
متن کاملThe Cliiord Paterson Lecture, 1995 Modelling Communication Networks, Present and Future
Modern communication networks are able to respond to randomly uctuating demands and failures by allowing buuers to ll, by rerouting traac and by real-locating resources. They are able to do this so well that, in many respects, large-scale networks appear as coherent, almost intelligent, organisms. The design and control of such networks present challenges of a mathematical, engineering and econ...
متن کاملThe role of the transverse carpal ligament in carpal stability: an in vitro study.
A biomechanical in vitro study was performed on 16 fresh frozen cadaver forearms to investigate the role of the transverse carpal ligament (TCL) in carpal stability. The distance between the scaphoid and hamate was measured, as a reference for the length of the TCL. Distances were recorded in both loaded and unloaded conditions after gradual sectioning of the transverse carpal ligament, the pal...
