Pneumopericardium: a Possible Rare Cause of Neonatal Death

چکیده
The aim of this paper is to point to the dangers of pneumopericardium (PPC), which is defined as a collection of air or gas in the pericardial cavity. PPC belongs to the most dangerous kinds of extra-alveolar air and only physicians' awareness of that condition and immediate air evacuation (pericardiocentesis) prevents the deleterious results. Neonatal pneumopericardium is a rare clinical condition. We report a case of neonatal pneumopericardium with the fatal outcome, occurring in the presence of significant lung pathology (in utero meconial aspiration, pulmonary hypertension) and a history of neonatal resuscitation. The patophysiological mechanism is the obstruction of large and small airways with the aspirated material and consecutive air trapping distal to obstruction, over distension and rupture of alveolar walls, allowing gas to escape and dissect along perivascular and peribronchial sheaths towards pericardial sac within which this air accumulates. Mechanically ventilated patients are particularly at risk for tension pneumopericardium (TPPC) secondary to barotrauma. Acute cardiac shock, pulsus paradoxus and decreased systolic blood pressure (the heart is ineffectual pump), muffled heart sounds and a metallic sound of high frequency refer to the presence of a pneumopericardium. Tension pneumopericardium is a life threatening condition, so early recognition and intervention (pericardiocentesis) is very important.

Neonatal Pneumopericardium in a Nonventilated Term Infant: A Case Report and Review of the Literature
Neonatal pneumopericardium (PPC) is a rare form of neonatal air leak syndrome with high morbidity and mortality. Air leak syndrome in the newborn is usually associated with active resuscitation, respiratory distress syndrome, meconium aspiration syndrome, mechanical ventilation, or trauma associated with labour. Neonatal PPC can be associated with other air leak syndromes such as pneumomediasti...

Background: Neonatal pneumopericardium (PPC) is a rare clinical condition usually associated with other air leak syndromes. It increases morbidity and mortality due to cardiac tamponade. Case report: A preterm male neonate weighing 1260 g was born with the gestational age of 28 weeks. Cardiopulmonary resuscitation was routinely performed without any medical therapy. The newborn was transferred ...
متن کاملA Rare Case of Isolated Pneumopericardium Developed Following a Penetrating Thoracic Trauma
Pneumopericardium indicates the free air in the pericardium. It can develop secondary to many different reasons and the most common cause in etiology is trauma. Other pathologies are usually accompanied in cases of pneumopericardium developed secondary to penetrating thoracic traumas, and isolated pneumopericardium is a very rare condition. In this article, a rare case of isolated pneumopericar...

INTRODUCTION Pneumopericardium, defined as the presence of gas in the pericardial sac, is a rare condition caused mostly by trauma. Tension pneumopericardium is a cause of hemodynamic instability; hence, it consists in a life-threatening situation and should be regarded in blunt chest trauma. CASE REPORT A 51-year-old male was victim of a 4m fall and burial. He was stable upon admission and p...
