Farewell to surgical mitral commissurotomy for many patients.

چکیده
Percutaneous mitral balloon valvotomy (PMV) has been accepted as an alternative to surgical mitral commissurotomy in the treatment of patients with symptomatic rheumatic mitral stenosis. Previous studies have demonstrated that PMV produces good immediate and long-term follow-up results in a selected group of patients with mitral stenosis.1-4 Hemodynamic and clinical improvement is achieved in the majority of patients with rheumatic mitral stenosis. PMV resulted in a significant decrease in mitral gradient and an increase in mitral valve area with minimal morbidity and mortality. The majority of patients have a marked clinical improvement, and the hemodynamic and clinical improvement produced by PMV persist at long-term follow-up.2-4 On the other hand, surgical mitral commissurotomy has been used successfully for many years to treat patients with mitral stenosis. The results of closed or open surgical mitral commissurotomy have demonstrated favorable immediate and long-term hemodynamic and symptomatic improvement in selected patients with rheumatic mitral stenosis.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...

متن کاملThe Association between B-type Natriuretic Peptide Family and Successful Percutaneous Transvenous Mitral Commissurotomy: A Systematic Review
B-type natriuretic peptide (BNP) level is known to increase in patients with rheumatic mitral stenosis. In this systematic review, we aimed to discuss the possible association between plasma BNP level and the success rate of percutaneous transvenous mitral commissurotomy. PubMed and Scopus databases were searched systematically, using the following key terms: “B-type natriuretic peptide” OR “BN...

PURPOSE To evaluate long-term survival and valve-related complications as well as prognostic factors for mid- and long-term outcome after closed mitral commissurotomy, covering a follow-up period of 14 years. MATERIAL AND METHODS Between 1989 and 2003, 36 patients (28 women and 8 men, mean age 28.8 +/- 6.1 years) underwent closed mitral commissurotomy at our institution. The majority of patie...
متن کاملPercutaneous balloon versus surgical closed commissurotomy for mitral stenosis.
BACKGROUND We performed a prospective, randomized trial comparing percutaneous balloon commissurotomy with surgical closed commissurotomy in 40 patients with severe rheumatic mitral stenosis. METHODS AND RESULTS Data were analyzed by investigators who were masked to treatment assignment or phase of study. Patients randomized to balloon (n = 20) or surgical (n = 20) commissurotomy had severe m...
