Marked elevation of the jugular venous pressure should raise the index of suspicion for constrictive pericarditis.

چکیده
Given the fact that, in the ‘constricting differential . . . ’, the successful management hinged crucially on the identification of the constrictive component of pericarditis, the latter component typically characterized by marked elevation of the jugular venous pressure (JVP), it would be useful to know how elevated that parameter was in the subject of the case report. In one series, 75% of 62 patients with constrictive pericarditis (CP) had an elevation in the JVP up to the angle of the jaw even when sitting up. In another series, comprising 25 patients with CP, 24 of whom had associated pleural effusion, the same degree of JVP elevation was noted in some of the subjects. Individual case reports also document this degree of elevation in JVP when CP is associated with pleural effusion. Second, given the fact that the JVP is elevated at a early stage of CP and is the last abnormal sign to disappear after successful pericardiectomy, was there a marked degree of elevation in JVP when the patient appeared to be responding to diuretics during the period antedating pericardiectomy? If that had been the case the index of suspicion for CP would have been even higher, given the fact that ‘a major clinical clue to the diagnosis (of CP) is the continued elevation of the central venous pressure after adequate diuresis’, the latter observation being borne out by 9 of the 11 cases reported by Conti and Friesinger. Conflict of interest: None declared.

Effusive-constrictive pericarditis, hepatitis, and pancreatitis in a patient with possible coxsackievirus B infection: a case report
BACKGROUND Coxsackie B is a viral pathogen that presents with various invasive diseases in adults. Historically, the majority of adult cases with pericarditis or myocarditis have been attributed to coxsackievirus B. The presentation of this viral infection causing effusive-constrictive pericarditis, hepatitis or pancreatitis is rare. This case report is the first to describe a patient with conc...
متن کاملConstrictive Pericarditis as a Never Ending Story: What's New?
Nowadays, we have a better understanding of the natural history of constrictive pericarditis such as transient constriction. In addition, we have acquired the correct understanding of hemodynamic features that are unique to constrictive pericarditis. This understanding has allowed us to diagnose constrictive pericarditis reliably with Doppler echocardiography and differentiation between constri...
متن کاملMajor Vessel Venous Thrombosis in Patients of Posttubercular Chronic Constrictive Pericarditis Undergoing Pericardectomy: A Rare Scenario
We are reporting two cases of neck and arm major venous thrombosis in patients of posttubercular chronic constrictive pericarditis posted for pericardectomy. There was unanticipated difficulty in placement of Internal Jugular vein catheter and subsequent ultrasound revealed thrombosis in the major veins. It was not diagnosed in the preoperative period. This report raises this major complication...
متن کاملValue of reference tracings in diagnosis and assessment of constrictive epi- and pericarditis.
Reference tracings are of great value in the diagnosis and assessment of constrictive pericarditis. The Q-h interval in the jugular venous pulse tracing is strongly correlated with the mean right atrial pressure (r=0.91). The left ventricular ejection time, the Q-A2 interval, and the Q-h interval are independent during atrial fibrillation from the preceding diastolic filling interval. This diff...
متن کاملKussmaul Physiology in Patients With Heart Failure
Elevated right atrial (RA) pressure is associated with renal impairment and increased mortality in patients with heart failure. Raised jugular venous pressure, as a bedside estimate of RA pressure, has similarly been shown to be associated with adverse outcome with increased heart failure hospitalizations and death. RA pressure (and jugular venous pressure) normally falls during inspiration des...
