چکیده

Methods Consecutive patients with paroxysmal AF in a randomised study comparing pulmonary vein isolation by wide area circumferential radiofrequency ablation (WACA) to ostial ablation with a cryo-balloon (CRYO) underwent CMR late gadolinium enhancement (LGE) imaging preand 3 months post ablation. Imaging was anonymized for blinded analysis of (1) LGE images, and (2) a 3D fusion image with LGE projected onto a segmented LA surface (thresholding set at 5 SD above mean ventricular signal). Scans were categorised using both assessment techniques separately as pre or post ablation, and if post ablation, whether lesions were in an ostial or WACA distribution. Results LGE imaging was performed in 50 patients (aged 61 ± 10 years, 72% male). Sensitivity and specificity for detection of ablation lesions was 60% and 96% on LGE imaging, or 88% and 82% respectively on 3D fusion imaging. Detection of WACA and CRYO lesion sets were correct in 14/24 and 16/26 respectively on LGE imaging, or 21/24 and 23/26 on 3D fusion imaging. Assessment of lesion distribution was correct for 13/50 from LGE images (4/24 for WACA and 9/26 for CRYO) and 26/50 from 3D fusion images (p = 0.013; 13/24 for WACA and 13/26 for CRYO).

Cardiovascular Magnetic Resonance Imaging of Scar Development Following Pulmonary Vein Isolation: A Prospective Study

AIMS Cardiovascular magnetic resonance (MR) provides non-invasive assessment of early (24-hour) edema and injury following pulmonary vein isolation (by ablation) and subsequent scar formation. We hypothesize that 24-hours after ablation, cardiovascular MR would demonstrate a pattern of edema and injury due to ablation and the severity would correlate with subsequent scar. METHODS Fifteen atri...

Asymptomatic cerebral lesions during pulmonary vein isolation under uninterrupted oral anticoagulation.

AIMS Left atrial radiofrequency ablation has been shown to carry a risk of asymptomatic cerebral lesions. No data exist in patients under continued oral anticoagulation during the ablation procedure. The aim of this study was to quantify the amount of silent cerebral lesions assessed by pre-procedural and post-procedural magnetic resonance imaging (MRI) in patients under therapeutic internation...

Role of Cardiac Imaging (CT/MR) Before and After RF Catheter Ablation in Patients with Atrial Fibrillation.

Pre-procedure X-ray computed tomography (CT) and magnetic resonance imaging (MRI) angiography are commonly used to delineate the complex and variable relationship of the left atrium, pulmonary veins, and surrounding structures. 3D CT and MR angiography are routinely incorporated into electroanatomic mapping systems to guide ablation lesion placement in the context of patient specific anatomy. P...

Visualization of pulmonary vein–left atrium lesions using delayed-enhancement magnetic resonance imaging after cryothermal balloon catheter ablation: A case report

Introduction Electrical pulmonary vein (PV) isolation is an effective technique that is used to treat patients with paroxysmal atrial fibrillation (AF). In recent years, PV isolation by cryothermal balloon (CB) ablation has been increasingly adopted by electrophysiology laboratories around the world. This technology is associated with more reproducible results and reduced procedural times and m...

Cerebral micro-embolization during pulmonary vein isolation: Relation to post-ablation silent cerebral ischemia.

BACKGROUND High incidences of silent cerebral ischemia (SCI) have been revealed by diffusion-weighted magnetic resonance imaging (DW MRI) after pulmonary vein isolation (PVI) for atrial fibrillation. A high number of mostly gaseous micro-embolic signals (MESs) was detected by transcranial Doppler (TCD) during PVI. In this investigation the possible relationship between MESs detected intraoperat...
