چکیده

Introduction: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder affecting adolescent children [1]. The etiology is thought to be multifactorial, related to both biological and biomechanical factors [2]. SCFE occurs when the epiphysis of the proximal femur slips in relation to the metaphysis through the growth plate, causing pain, disability and potential long-term sequellae from joint incongruity. The current standard treatment for SCFE is percutaneous fixation using a single steel screw with the goal of preventing further slippage and obtaining early physeal closure. Bone allograft epiphyseodesis is an alternative treatment method in which a beam-shaped freeze-dried cortical allograft is placed percutaneously into a drill hole through the cortex, up the femoral neck and into the femoral head [3]. Allograft epiphysiodesis avoids the need for a second surgery to remove the hardware, but has the disadvantage that it cannot achieve stability until the allograft is incorporated into the surrounding bone. We hypothesized that a threaded cortical bone allograft could provide sufficient initial stability to serve as a viable alternative to the steel implant. The objective of this study was to compare the fixation strength of a single threaded bone allograft versus a single steel screw in an in vitro pig model of SCFE.

Torsional Strength of Single–versus Double–screw Fixation in a Model of Unstableslipped Capital Femoral Epiphysis

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder of adolescent children [1]. It results in fracture of the growth cartilage, separating the proximal femoral epiphysis from the metaphysis, due to hormonal and mechanical factors [2]. The most widely accepted treatment for stable SCFE (defined by the ability to bear weight on the affected limb) has been a sing...

Progressive slip after removal of screw fixation in slipped capital femoral epiphysis: two case reports

UNLABELLED INTRODUCTION In slipped capital femoral epiphysis the femoral neck displaces relative to the head due to weakening of the epiphysis. Early recognition and adequate surgical fixation is essential for a good functional outcome. The fixation should be secured until the closure of the epiphysis to prevent further slippage. A slipped capital femoral epiphysis should not be confused wit...

Bilateral epiphyseal migration following fixation for slipped capital femoral epiphyses in a hypothyroid child.

Progression of slipped capital femoral epiphysis following in situ screw fixation typically occurs through loosening of the screw in the metaphysis. Epiphyseal migration off the screw due to physeal growth is rare. We report epiphyseal migration off bilateral screws in a child undergoing thyroid replacement therapy. Patients with mild and moderate slipped capital femoral epiphysis and endocrine...

Surgical Technique on screw removal in case of broken screw in SCFE Introduction Slipped Capital Femoral Epiphysis

We describe a case where a traumatic unstable SCFE that was initially treated with closed reduction and fixation with a single transphyseal screw went on to hardware failure with recurrence of the deformity. The complication was successfully treated with closed reduction and recannulating the fractured screw within the epiphysis and extracting it using a conical extraction screw commonly referr...

Intra-operative arthrography facilitates accurate screw fixation of a slipped capital femoral epiphysis.

We report the case of a 13-year-old obese child presenting with bilateral stable slipped capital femoral epiphyses, which were managed by percutaneous single screw fixation in situ under image intensifier control using arthrography.
