چکیده

In the late 1990s the development of endoscopic submucosal dissection (ESD) using different knives revolutionized the spectrum of endoscopic resection techniques allowing en bloc resections regardless of the lesion’s size [5] . In combination with new data on the risk of lymph node metastases in early gastric cancer resection, criteria could be extended [6, 7] . The so-called expanded criteria include mucosal cancers without ulceration regardless of the lesion’s size, ulcerated mucosal cancers with a size restriction of 30 mm, and cancers with minute submucosal invasion of ! 500 m (also with a size restriction of 30 mm). Lesions with undifferentiated histology and lymphatic or vascular invasion are still excluded [7, 8] . In 2006 Oka et al. [9] confirmed the presumed advantages of the ESD method in comparison to EMR. ESD showed a significant improvement in en bloc resections and complete resections. For lesions without ulceration, the superiority of ESD was significant even for small lesions of

Endoscopic resection of gastric and esophageal cancer

Endoscopic submucosal dissection (ESD) and endoscopic mucosal resection (EMR) techniques have reduced the need for surgery in early esophageal and gastric cancers and thus has lessened morbidity and mortality in these diseases. ESD is a relatively new technique in western countries and requires rigorous training to reproduce the proficiency of Asian countries, such as Korea and Japan, which hav...

Endoscopic Submucosal Dissection for Early Gastric Cancers with Uncommon Histology

Endoscopic submucosal dissection (ESD) enables en bloc curative resection of early gastric cancers (EGCs) with a negligible risk of lymph node metastasis (LNM). Although ESD for EGCs with absolute and expanded indications is safe, the results differ between EGCs with specialized and common histologies. EGC with papillary adenocarcinoma is a differentiated-type adenocarcinoma. At present, it is ...

Long-Term Clinical Efficacy and Perioperative Safety of Endoscopic Submucosal Dissection versus Endoscopic Mucosal Resection for Early Gastric Cancer: An Updated Meta-Analysis

Background To systematically evaluate the safety and efficacy of endoscopic submucosal dissection (ESD) versus endoscopic mucosal resection (EMR) for early gastric cancer (EGC). Methods We searched the databases of PubMed, Web of Science, EMBASE, and the Cochrane Library from January 2000 to April 2017 and included studies that compared the outcomes of ESD with EMR for EGC. These eligible stu...

Global dissemination of endoscopic submucosal dissection for early gastric cancer.

Gastric cancer is one of the most common cancers. Early detection is critical for its curative treatment. Endoscopic mucosal resection (EMR) is accepted as a treatment of choice for differentiated early gastric cancer (EGC). Recently, Gotoda et al. provided important information on the risks of lymph node metastasis in a large EGC series (1). Based on their analyses, the expanded criteria for e...

Surveillance strategy based on the incidence and patterns of recurrence after curative endoscopic submucosal dissection for early gastric cancer.

BACKGROUND AND STUDY AIMS To suggest an appropriate surveillance strategy after curative endoscopic submucosal dissection (ESD) for early gastric cancers, based on incidence and patterns of local, metachronous, and extragastric recurrence. PATIENTS AND METHODS Between 2003 and 2011, 1497 consecutive patients with 1539 differentiated-type early gastric cancers meeting absolute or expanded indi...
