A Convertible Cardiopulmonary Bypass System for Optimized Hemofiltration

چکیده
Abstrad------------------Hemofiltration during or after cardiopulmonary bypass has proved to be an efficient tool for hemoconcentration. Classically, the hemofilters are supplied by the arterial line and the filtration rate is mainly a function of the perfusion pressure. We have developed a convertible cardiopulmonary bypass system including a double head roller pump for blood cardioplegia application that allows optimized hemofiltration independent from systemic perfusion. Serial bench studies were performed at normothermia with a mean hematocrit of 20%, and identical hemofilters:

BACKGROUND Activation of the inflammatory cascade is thought to account for some of the respiratory dysfunction and prolonged mechanical ventilation associated with cardiopulmonary bypass. The objective of this investigation was to identify whether perioperative steroids or hemofiltration during cardiopulmonary bypass, by their attenuation of inflammation, would reduce duration of mechanical ve...
متن کاملAggressive zero balance ultrafiltration on CPB in patients with renal failure may cause cerebral edema: a theoretical analysis.
The objective of this study was to determine the brain volume changes that occur secondary to hemofiltration during cardiopulmonary bypass in patients with renal failure. We hypothesized that in patients with elevated urea levels, quick aggressive hemofiltration could be associated with cerebral edema. We constructed a simple two-compartment model similar to the urea kinetic model developed by ...

Ultrafiltration has proven to be an effective method of reducing post-cardiopulmonary bypass edema and the inflammatory response associated with activation of the complement cascade. Recently, a modified ultrafiltration process has been developed which allows further hemoconcentration after weaning from bypass. Both of these procedures can have an effect upon the concentrations of many of the p...
متن کاملThe effects of early high-volume hemofiltration on prolonged cardiac arrest in rats with reperfusion by cardiopulmonary bypass: a randomized controlled animal study
BACKGROUND It is not yet clear whether hemofiltration can reduce blood cytokine levels sufficiently to benefit patients who suffer prolonged cardiac arrest (CA) treated with cardiopulmonary bypass (CPB). We sought to assess effects of high-volume and standard volume continuous veno-venous hemofiltration (CVVH) on blood cytokine levels and survival in a rat model of prolonged CA treated with CPB...
