Bilateral Tuberculous Pleural Effusions

چکیده
Mycobacterium tuberculosis remains a pathogen of con¬ siderable clinical and public health importance. Clas¬ sically, tuberculous pleuritis was regarded as a disease ofyoung adults characterized by a unilateral, lymphocytic exudate with a self-limited short-term course. In the last two decades, how¬ ever, there has been increasing recognition that tuberculous pleuritis is occurring in an older patient population, with reac¬ tivation as well as primary disease, and that its features often vary from this classic picture. We report the case of an elderly man with bilateral tuberculous pleural effusions.one lympho¬ cytic and the other persistently neutrophilic.

Background & Aim: Adenosine deminase(ADA) can be used in the diagnosis of tuberculous pleural effusions. In countries with high prevalence of tuberculous pleural effusions, specificity and sensitivity for ADA test is high, therefore it is an integral part of a diagnostic workup of lymphocyte-rich exudative body fluids, and it is a cheap and economically cost-effective test. No study has bee...
متن کاملTh1 and Th2 Cytokine Profiles in Malignant Pleural Effusion
Background: The alteration of Th1 and Th2 cytokine levels is the subject of controversy in pleural effusions caused by malignancy, a situation that favors a Th2 immune response. Objective: To examine the different levels of IL-4 and IL-10 (Th2 cytokines), and IL-2 and interferon-γ (IFN-γ) (Th1 cytokines) in malignant and non-malignant pleural effusions. Method: The cytokine levels in pleural fl...
متن کاملTh1/Th2 Imbalance and Elevated PD-L1 in Pleural Effusion Predict the Risk of Multi-Drug Resistant Tuberculous Pleuritis
Background: Patient immune status might be indicative of the variance in bacterial genetics in drug-resistant tuberculous pleuritis and could be used for predicting the risk of multi-drug resistant tuberculous pleuritis (MDR-TB). Objective: To determine the significance of Th2/Th1 ratio and concentration of PD-L1 in the pleural effusions for prediction of MDR-T...

Aim: The aim of this study is to assess the usefulness of Adenosine deaminase levels of the pleural fluid in three different groups of patients and to confirm its usefulness in the diagnosis of tuberculous effusions. Materials and methods: 25 consecutive patients admitted with their first episode of pleural of were included in the study. The routine investigations were history, clinical examina...
متن کاملRole of Adenosine Deaminase Estimation in Differentiation of Tuberculous and Non-tuberculous Exudative Pleural Effusions
BACKGROUND Tuberculosis kills five lakh patients every year in India, commonest being pulmonary tuberculosis and is often associated with effusion. Delay in diagnosis and treatment results in poor prognosis. Several studies have suggested the role of adenosine deaminase (ADA) in the diagnosis of tuberculous pleural effusions, but false-positive results from lymphocytic effusions have also been ...
