What are the most appropriate return-to-play guidelines for concussed child athletes?

چکیده
OBJECTIVE To examine concussion literature for specific guidelines regarding return to play (RTP) following sport-related concussion in child athletes. To make recommendations regarding the most appropriate RTP guidelines for child athletes following sport-related concussion. DESIGN A literature review of concussion literature. INTERVENTION A literature search was conducted using Medline and Embase databases from 1998 to 2008. More than 60 articles and two websites were reviewed. RESULTS There is a paucity of research on sport-related concussion in child athletes, particularly younger children (age 5-12 years). In particular, there is no research on RTP guidelines for child athletes following sport-related concussion. Child athletes take longer to recover from concussions than adults. Concussion symptoms may resolve before cognitive function has completely recovered. Concussion assessment and management in children can be confounded by their growth and development, as well as the lack of trained medical personnel involved with youth sports. There are no child-specific assessment tools for concussion. CONCLUSIONS RTP decisions in children should be made cautiously and should be individualised. No concussed child athlete should be allowed to RTP the same day. Physical and cognitive rest is very important to allow for the resolution of concussion symptoms. Child athletes should remain symptom free for several days before starting a medically supervised stepwise exertion protocol. Further research is needed to elucidate the effects of concussion in children and to determine the most appropriate RTP guidelines. Child-specific concussion assessment tools need to be developed to improve concussion assessment and management in children.

Are athletes psychologically ready for sport following a concussion?
Sport-related concussions have garnered increasing attention in recent years. Part of making sport safer at all levels of competition involves ensuring that concussions are properly managed. Graduated return to play (RTP) has been a widely implemented strategy to assist with concussion management. RTP is a six-stage process managed by health professionals that progressively increases athletes’ ...
متن کاملNeurocognitive performance of concussed athletes when symptom free.
CONTEXT Concussed athletes may underreport concussion-related symptoms in order to expedite return to play. Whether neurocognitive impairments persist once concussion-related symptoms resolve has yet to be determined. Reliance on athlete-reported, postconcussion symptoms when making return-to-play decisions may expose athletes to subsequent injury if complete recovery has not occurred. OBJECT...
متن کاملConcussion Increases Odds of Sustaining a Lower Extremity Musculoskeletal Injury After Return to Play Among Collegiate Athletes.
BACKGROUND Previous studies have identified abnormalities in brain and motor functioning after concussion that persist well beyond observed clinical recovery. Recent work suggests subtle deficits in neurocognition may impair neuromuscular control and thus potentially increase risk of lower extremity musculoskeletal injury after concussion. PURPOSE To determine the odds of sustaining an acute ...

OBJECTIVE To examine the ethical and legal issues physicians face when evaluating and managing athletes with sports-related concussions, and to offer guidance to physicians as they navigate these situations. RESULTS This position paper reviews and compares the components of sports-related concussion laws, including education, removal from play, and clearance for return to play. It highlights ...
متن کاملFunctional brain abnormalities are related to clinical recovery and time to return-to-play in athletes.
OBJECTIVE The relationship between athlete reports of symptoms, neurophysiological activation, and neuropsychological functioning is investigated in a sample of high school athletes. METHODS All athletes were evaluated using functional magnetic resonance imaging (fMRI), a computer-based battery of neurocognitive tests, and a subjective symptom scale. Athletes were evaluated within approximate...
