Left ventricular remodeling after reperfused acute myocardial infarction: insights from automated ECV mapping
نویسندگان
چکیده
Background In reperfused acute ST-segment elevation myocardial infarction (STEMI), infarct size and microvascular obstruction (MVO) are strong predictors of adverse LV remodeling and poor outcome. However, the role of remote myocardium is less clear. Remote native T1 changes have been linked with adverse LV remodeling and is thought to be associated with inflammation. We used automated ECV mapping to provide further insights into the pathophysiology of LV remodeling.

Automated Extracellular Volume Fraction Mapping Provides Insights Into the Pathophysiology of Left Ventricular Remodeling Post–Reperfused ST‐Elevation Myocardial Infarction
BACKGROUND Whether the remote myocardium of reperfused ST-segment elevation myocardial infarction (STEMI) patients plays a part in adverse left ventricular (LV) remodeling remains unclear. We aimed to use automated extracellular volume fraction (ECV) mapping to investigate whether changes in the ECV of the remote (ECVR emote) and infarcted myocardium (ECVI nfarct) impacted LV remodeling. METH...
متن کاملChanges in remote myocardial tissue after acute myocardial infarction and its relation to cardiac remodeling: A CMR T1 mapping study
OBJECTIVES To characterize the temporal alterations in native T1 and extracellular volume (ECV) of remote myocardium after acute myocardial infarction (AMI), and to explore their relation to left ventricular (LV) remodeling. METHODS Forty-two patients with AMI successfully treated with primary PCI underwent cardiovascular magnetic resonance after 4-6 days and 3 months. Cine imaging, late gado...
متن کاملExtra-cellular expansion in the normal, non-infarcted myocardium is associated with worsening of regional myocardial function after acute myocardial infarction
BACKGROUND Expansion of the myocardial extracellular volume (ECV) is a surrogate measure of focal/diffuse fibrosis and is an independent marker of prognosis in chronic heart disease. Changes in ECV may also occur after myocardial infarction, acutely because of oedema and in convalescence as part of ventricular remodelling. The objective of this study was to investigate changes in the pattern of...
متن کاملLeft atrial volume during the early convalescent phase of acute MI is strongly related to expansion of myocardial extracellular matrix during infarct healing and ventricular remodeling
Background Changes in left ventricular compliance caused by diffuse fibrosis after MI may result in diastolic dysfunction and left atrial enlargement. We sought to test the hypothesis that left atrial volume is associated with ECV (a marker of diffuse myocardial fibrosis) during the sub-acute phase of infarction and can predict the increase of ECV during the ensuing months of infarct healing. W...
متن کاملRemote Zone Extracellular Volume and Left Ventricular Remodeling in Survivors of ST-Elevation Myocardial Infarction
UNLABELLED The natural history and pathophysiological significance of tissue remodeling in the myocardial remote zone after acute ST-elevation myocardial infarction (STEMI) is incompletely understood. Extracellular volume (ECV) in myocardial regions of interest can now be measured with cardiac magnetic resonance imaging. Patients who sustained an acute STEMI were enrolled in a cohort study (BHF...
