چکیده

W analyze the effect of tumor repopulation on optimal dose delivery in radiation therapy. We are primarily motivated by accelerated tumor repopulation toward the end of radiation treatment, which is believed to play a role in treatment failure for some tumor sites. A dynamic programming framework is developed to determine an optimal fractionation scheme based on a model of cell kill from radiation and tumor growth in between treatment days. We find that faster tumor growth suggests shorter overall treatment duration. In addition, the presence of accelerated repopulation suggests larger dose fractions later in the treatment to compensate for the increased tumor proliferation. We prove that the optimal dose fractions are increasing over time. Numerical simulations indicate a potential for improvement in treatment effectiveness.

Tumour radiobiology beyond fractionation

Historically it has been shown repeatedly that single high doses of radiation do not allow a therapeutic differential between tumor and critical normal tissues but dose fractionation does. The purpose of conventional dose fractionation is to increase dose to the tumor while preserving normal tissue function. Tumors are generally irradiated with 2Gy dose per fraction delivered daily to a more or...

Dynamic optimization of fractionation schedules in radiation therapy

In this thesis, we investigate the improvement in treatment effectiveness when dynamically optimizing the fractionation scheme in radiation therapy. In the first part of the thesis, we consider delivering a different dose each day depending on the observed patient anatomy. Given that a fixed prescribed dose must be delivered to the tumor over the course of the treatment, such an approach result...

Energy Optimization And Calculation Of Dose Absorption Enhancement Factor In Photon Activation Therapy

Introduction: Secondary radiation such as photoelectrons, Auger electrons and characteristic radiations cause a local boost in dose for a tumor when irradiated with an external X-ray beam after being loaded with elements capable of activating the tumor, e.g.; I and Gd. Materials and Methods:  In this investigation, the MCNPX code was used for simulation and calculation of dose enhancement facto...

Altered fractionation for head and neck cancer.

A conventional course of radiation for squamous cell carcinoma in the United States is generally 70 Gy in 7 weeks, with a once-daily dose of 1.8 to 2 Gy. This schedule has a modest success rate in curing head and neck cancer. The past several decades have seen numerous investigations into altering this schedule to optimize the results of radiation. Two approaches, founded on radiobiologic princ...

Dosimetric comparison of treatment of left breast cancer using two different dose schedules Dosimetric comparison of treatment of left breast cancer using two different dose schedules

Background: To date, different kinds of treatment methods have been proposed for radiotherapy of cancer patients. Choosing the kind of treatment method affects the quality of the patient's treatment. This study aims to investigate the effect of the number of radiation treatment sessions on the dose received by the patient and the distribution of tumor dose and dose received by organs at risk in...
