MORE THAN 25 years ago, C.P. Bailey performed the first successful left ventricular aneurysmectomy in a human being.' Since then, with cardiopulmonary bypass and improved surgical techniques, aneurysmectomy has become an established procedure available to patients with ventricular aneurysms and severe symptoms. In this paper we review the available literature concerned with elective left ventricular aneurysmectomy in the setting of chronic ischemic heart disease. We compare theoretical models and predictions with the empirical observations made by previous investigators and suggest an approach to the symptomatic patient with a left ventricular aneurysm.
