Diffuse Esophageal Spasm

چکیده
An 82-year-old man was admitted to our hospital due to dysphagia. Barium esophagography showed the esophagus to have a corkscrew appearance (Picture 1). Esophagogastroduodenoscopy also showed similar findings (Picture 2). Given these typical imaging findings, he was diagnosed with diffuse esophageal spasm (DES), although esophageal manometry for a definite diagnosis could not be performed due to the patient’s refusal. Proton pump inhibitors and calcium channel blockers were unable to improve his symptoms. However, he decided not to undergo further medical intervention such as endoscopic myotomy or balloon dilatation because switching his diet from solid foods to minced meal alleviated his dysphagia. DES is a rare esophageal motility disorder accompanied by dysphagia (1). Although it has been suggested that disordered neural inhibition caused by nitric oxide deficiency might be involved in the pathogenesis of DES, its mechanisms remain unclear. Striking “corkscrew” imaging can help diagnose DES.

Of 130 patients diagnosed to have esophageal motility disorders, transformation of diffuse esophageal spasm (DES) into achalasia was identified in one patient. The clinical syndmme of DES was first described by Hamilton Osgood in 1889. It is characterizedby symptoms of substernal chest pain and dysphagia, tertiary contractions on barium swallow and a manometric pattern of frequent simultaneous ...

متن کاملAchalasia and diffuse oesophageal spasm in siblings.
We report the occurrence of achalasia and diffuse oesophageal spasm in brother and sister. The familial occurrence of these unusual disorders is consistent with the possibility that they share a common aetiology and indicates the potential importance of genetic predisposition.
