Angiotensin-Converting Enzyme Inhibition and Parathyroid Hormone Secretion
نویسندگان
چکیده
BACKGROUND Prior studies suggest that renin-angiotensin-aldosterone system (RAAS) inhibitors decrease parathyroid hormone (PTH) secretion. OBJECTIVE To evaluate the effect of angiotensin-converting enzyme inhibitors (ACEi) on serum PTH in participants with and without primary hyperparathyroidism (P-HPT). METHODS An open-label, single-arm, pilot study whereby participants with and without P-HPT had PTH were evaluated before and after 1 week of maximally tolerated lisinopril therapy. RESULTS A total of 12 participants with, and 15 participants without, P-HPT successfully completed the protocol. Following 1 week of lisinopril, participants with P-HPT had a decrease in systolic blood pressure (SBP) (-6.4 mmHg, P < 0.01), an increase in plasma renin activity (PRA) (+1.50 ng/mL/h, P = 0.06), and a decrease in PTH (79.5 (21.6) to 70.9 (19.6) pg/mL, ∆ = -8.6 pg/mL, P = 0.049); however, serum and urine calcium did not change. In contrast, although 1 week of lisinopril significantly decreased SBP and increased PRA among participants without P-HPT, there were no changes in PTH or calcium. CONCLUSION In this short pilot investigation, 1 week of maximally titrated ACEi did not impact PTH in participants without P-HPT, but resulted in a modest and marginally significant reduction of PTH but not calcium, among participants with P-HPT. This trial is registered with ClinicalTrials.gov NCT01691781.

Attenuation of Focal Cerebral Ischemic Injury Following Post-Ischemic Inhibition of Angiotensin Converting Enzyme (ACE) Activity in Normotensive Rat
Background: Central renin angiotensin system has an important role on the cerebral microcirculation and metabolism. Our previous work showed that inhibition of angiotensin converting enzyme (ACE) activity prior to induction of ischemia protected the brain from severe ischemia/reperfusion (I/R) injuries. This study evaluated the impacts of post-ischemic inhibition of ACE, enalapril, on brain inf...
متن کاملInhibition of Angiotensin-Converting Enzyme Reduces Cerebral Infarction Size in Experimental-Induced Focal Cerebral Ischemia in the Rat
Background: The role of Renin Angiotensin System (RAS) in ischemic/reperfusion (I/R) injuries is not fully elucidated. Furthermore, it is not clear whether inhibition of RAS by Angiotensin-Converting Enzyme (ACE) inhibitors has beneficial effects in terms of protecting the brain from I/R injuries. In this study enalapril is used as an ACE inhibitor to evaluate the role of RAS in I/R injuries in...
متن کاملSerum Angiotensin Converting Enzyme in Patients with Psoriasis
Background: Controversial data concerning the elevation of serum angiotensin-converting enzyme in psoriasis are reported in the literature. In order to verify whether this abnormality exists in Iranian patients, we performed this study. Method: Serum angiotensin-converting enzyme level was measured in 40 psoriatics. According to clinical forms of psoriasis, patients were further divided into th...
متن کاملEffects of acute ACE inhibition on pulsatile renin and aldosterone secretion and their synchrony.
Pulsatile (burstlike) secretion of renin and aldosterone is positively coupled with a short time lag of about 10 to 20 minutes. We investigated how acute interruption of the renin-angiotensin-aldosterone axis, ie, acute angiotensin-converting enzyme (ACE) inhibition, alters the pattern of renin and aldosterone secretion. Eight healthy men (mean age, 22+/-1 years) were studied while on standardi...
متن کاملPotential angiotensin converting enzyme (ACE) inhibitors from Iranian traditional plants described by Avicenna’s Canon of Medicine
Objective: Hypertension is an important cause of cardiovascular disorders. The angiotensin converting enzyme (ACE) plays an important role in hypertension; therefore, inhibition of ACE in treatment of chronically elevated blood pressure is an important therapeutic approach. In the current review, we have p...
