Physical health indicators in SMI.

چکیده
Abbt and Alderson remark ‘how little time we spend in general practice ... perhaps 8 weeks over an entire degree’ but at Imperial College London and probably at most other medical schools, this represents greater time allotted than for many other specialties. We suggest the focus in medical schools should shift towards improving the quality of general practice placements and promoting the integration of primary care and specialist teaching, rather than consuming more time in an already overstretched curricula. We feel that prestige has never been the main incentive for pursuing a specialty. Our own experience is that many medical students are attracted to a career in general practice because of other factors, such as a good work–life balance, continuity of care and career flexibility. With many GPs now concerned about their workload, this inevitably influences students and junior doctors in their career choices.2 Another key factor is the funding that a specialty receives: recently, the proportion of the NHS budget spent on primary care has decreased and the income of GPs has fallen. To recruit more GPs, medical schools should improve the quality of students’ experiences in their primary care placements. However by itself, this will not be sufficient to improve recruitment and the onus falls upon the NHS to once again make general practice a rewarding career for doctors.

Identifying primary care quality indicators for people with serious mental illness: a systematic review
BACKGROUND Serious mental illness (SMI) - which comprises long-term conditions such as schizophrenia, bipolar disorder, and other psychoses - has enormous costs for patients and society. متن کاملHealth-Related Quality of Life and Overall Life Satisfaction in People with Serious Mental Illness
Quality of life (QoL) in people with schizophrenia and other serious mental illnesses (SMI) is an important outcome goal, yet there is no consistent definition of the construct. We examined three aspects of QoL in persons with SMI: overall life satisfaction, physical health-related QoL (HRQoL), and mental HRQoL. This study had two primary aims: first, to examine whether there are differences in...
متن کاملFinancial incentives improve recognition but not treatment of cardiovascular risk factors in severe mental illness
OBJECTIVES Severe mental illness (SMI) is associated with premature cardiovascular disease, prompting the UK primary care payment-for-performance system (Quality and Outcomes Framework, QOF) to incentivise annual physical health reviews. This study aimed to assess the QOF's impact on detection and treatment of cardiovascular risk factors in people with SMI. METHODS A retrospective open cohort...
متن کاملPatterns of physical co-/multi-morbidity among patients with serious mental illness: a London borough-based cross-sectional study

There may be variation within the SMI group in the distribution of physical co/multi-morbidity. This study aims to a) compare the pattern of physical co- and multi-morbidity between patien...
متن کاملA well-being programme in severe mental illness. Baseline findings in a UK cohort
INTRODUCTION Patients with severe mental illness (SMI) have higher rates of cardiovascular disease (CVD) morbidity and mortality than the general population. In the UK, data were limited regarding the known prevalence of physical health screening of SMI patients. AIMS A total of 966 patients with SMI from seven geographically varied regions in the UK agreed to participate in a 2-year nurse-le...
متن کاملPTSD'S mediation of the relationships between trauma, depression, substance abuse, mental health, and physical health in individuals with severe mental illness: evaluating a comprehensive model.
OBJECTIVE Following trauma exposure and PTSD, individuals with severe mental illness (SMI) frequently suffer a complex course of recovery complicated by reduced mental and physical health and increased substance abuse. The authors evaluated a theoretical PTSD-SMI model which theorizes that trauma, PTSD, depression, substance abuse, mental health, and physical health are interrelated and that PT...
