چکیده

CD: Crohn’s disease INTRODUCTION Crohn’s disease (CD) is an idiopathic inflammatory bowel disease that is characterized by noncaseating granulomas that occur in the gastrointestinal tract. This disease is chronic, incurable, and prone to relapse. CD is frequently associated with skin and mucous membrane lesions. The skin is among the most common sites for extraintestinal involvement in CD. Estimated incidence ranges from 22% to 44% and includes distant cutaneous (metastatic) CD, contiguous perianal CD, oral CD, reactive skin findings (including erythema nodosum and pyoderma gangrenosum), and nutritional skin changes. The site of involvement can influence lesion morphology. Contiguous cutaneous CD distinguishes from metastatic CD, as it extends directly from the gastrointestinal tract. Lesions that appear on the legs often present as intact inflammatory papules and plaques, whereas those involving intertriginous and genital skin usually ulcerate owing to the propensity for maceration in these sites. We describe a severe and extensive case of CD with perianal involvement contiguous to an anal CD lesion.

Medical Therapy of Perianal Crohn's Disease

BACKGROUND Development of perianal fistulas are a common feature of Crohn's disease (CD). Consequences are severe impairment of quality of life as well as potentially life-threatening complications like abscess formation or bacterial sepsis. Therefore, appropriate treatment is an important task in the management of CD. METHODS This review describes the epidemiology, pathogenesis, diagnosis, a...

Medical and surgical management of perianal Crohn’s disease

Crohn's disease is increasingly thought to encompass multiple possible phenotypes. Perianal manifestations account for one such phenotype and represent an independent disease modifier. In its more severe form, perianal Crohn's disease confers a higher risk of a severe and disabling disease course, relapses, hospital admissions and operations. This, in turn, imposes a considerable burden and dis...

Perianal complete remission with combined therapy (seton placement and anti-TNF agents) in Crohn's disease: a Brazilian multicenter observational study.

BACKGROUND Perianal fistulizing Crohn's disease is one of the most severe phenotypes of inflammatory bowel diseases. Combined therapy with seton placement and anti-TNF therapy is the most common strategy for this condition. OBJECTIVES The aim of this study was to analyze the rates of complete perianal remission after combined therapy for perianal fistulizing Crohn's disease. METHODS This wa...

Perianal Crohn’s disease should always be suspected in children and adolescents presenting with perianal skin manifestations to any healthcare settings in order to avoid delay in diagnosis and treatment of this chronic disab...

Fistulizing Crohn's Disease Presenting After Surgery on a Perianal Lesion.

Perianal skin lesions, such as skin tags, can be an early presenting sign of Crohn's disease. Surgical intervention on these lesions may increase the risk of fistula development and lead to worse outcomes. This case series examined 8 patients who underwent surgical intervention on what appeared to be benign perianal skin lesions, only to reveal fistulas leading to the diagnosis of Crohn's disea...
