چکیده

Early diagnosis and prompt treatment of gastrooesophageal reflux (GOR) can halt unnecessary investigations and prevent medical complications, such as failure to thrive,1 severe oesophagitis,2 and aspiration syndromes.3 The standard investigation for GOR is the upper gastrointestinal barium contrast series. However, because of poor sensitivity and specificity the role of this test in clinical practice has been questioned.4 Several other methods of investigation are available and to some extent the choice of investigation depends on the clinical problem.5 Over the last 10 years, 24 hour lower oesophageal pH monitoring has increasingly become established as the 'gold standard' for documenting 'acid' GOR.3 5 The ability to monitor over prolonged time intervals and the avoidance of any radiation are particularly attractive features of this test. However, until recently, the use of the technique has largely been confined to clinical research studies. At the Royal Hospital for Sick Children (RHSC), Glasgow, over the last four years an open clinical pH monitoring service has been available and has proved valuable in diagnosing and managing GOR. We describe here the development of this service, our experience with the technique, and our current practice.

Simultaneous recordings of oesophageal acid exposure with conventional pH monitoring and a wireless system (Bravo).

OBJECTIVES Oesophageal pH monitoring is a useful test for the diagnosis of gastro-oesophageal reflux disease (GORD) but has some limitations related to the nasopharyngeal electrode. Recently, a telemetric catheter free system (CFS) (Bravo; Medtronic) was developed. The aim of this study was to determine the concordance of data between the conventional pH measurement system (CPHMS) and the CFS B...

Ambulatory 24 hour intraesophageal pH-monitoring in the diagnosis of gastroesophageal reflux disease.

The results of ambulatory 24 hour oesophageal pH monitoring in 20 patients with established gastro-oesophageal reflux disease were compared with those of 20 healthy individuals with normal endoscopy. Cut off limits of pH 3, 4, and 5 were superior to pH 2 with respect to the discrimination of patients from normal subjects, and for the detection of pathological reflux. Using pH 4 as a cut off lim...

Exertional gastro-oesophageal reflux: a mechanism for symptoms in patients with angina pectoris and normal coronary angiograms.

During 24 hour oesophageal pH monitoring 52 patients who had angina pectoris and normal coronary angiograms underwent exercise testing, as far as their symptoms allowed, on a treadmill to determine whether gastro-oesophageal reflux occurred during exertion. In 11 patients the 24 hour oesophageal pH score was abnormally high; 10 of these showed exertional gastro-oesophageal reflux, and in nine t...

Paediatric gastro-oesophageal reflux: prognostic indicators from pH monitoring.

The original diagnostic 24 hour pH monitoring data in 57 children with gastro-oesophageal reflux (GOR) were retrospectively reviewed after a minimum of one year follow up. The tracings of children who responded to medical treatment were compared with those who failed to respond and required a fundoplication. Children with GOR secondary to oesophageal atresia/tracheo-oesophageal fistula and neur...

Patients with symptoms of delayed gastric emptying have a high prevalence of oesophageal dysmotility, irrespective of scintigraphic evidence of gastroparesis

Background Patients with symptoms suggestive of gastroparesis exhibit several symptoms, such as epigastric pain, postprandial fullness, bloating and regurgitation. It is uncertain if such symptoms reflect underlying oesophageal motor disorder. Aims To examine whether patients with epigastric pain and postprandial distress syndrome suggestive of functional dyspepsia and/or gastroparesis also h...
