Unreported Ipsilateral Injuries Concurrent with Perilunate Dislocation:a Case Report

A 16 year old, right hand dominant worker sustained a severe injury after his shirt had been caught by a kind of a roller. On evaluation, he had complaints of numbness and paresthesias in the hand with diffuse tenderness, swelling and deformities of the wrist, forearm and the arm. Circulation seemed to be impaired, but radial and ulnar pulses could be palpated, although with difficulty because of marked swelling. There was not any open injury or any kind of skin lesion. Radiographic examination revealed a dorsal transscaphoid perilunate fracture-dislocation together with the so called 'chip fractures' of the ulnar and radial styloid, as well as ipsilateral incomplete distal and complete diaphyseal fractures of the radius, and incomplete two level fracture of the ulna and diaphyseal humerus fracture (fig-1a,b,c,d).
