Primary transmission of multidrug-resistant and extensively drug-resistant tuberculosis among HIV-infected persons: what does the future hold in store?

چکیده
The article by Andrews et al. demonstrating exogenous reinfection as a cause of multidrug-resistant (MDR) and extensively drug resistant (XDR) tuberculosis (TB) in South Africa in the current issue of the Journal brings chilling news [1]. The authors report that all of the MDR and XDR TB that occurred in patients previously treated for TB was the result of primary transmission, rather than emerging as a result of inappropriate or inadequate treatment. All 17 of the subjects who they identified with MDR and/or XDR TB after an episode of drug-susceptible TB had been reinfected, and none of the cases were relapses with acquired drug resistance. This striking preponderance of reinfection suggests that exposure to and primary infection with MDR and/or XDR TB is the predominant mode of acquiring MDR and/or XDR TB in this population. This finding indicates that efforts to strengthen

Study of HIV seroprevalence in Pulmonary Tuberculosis Patients with Special Reference to Multidrug Resistant Mycobacteria
This study was planned to determine HIV seroprevalence among pulmonary tuberculosis patients, to characterize the isolated mycobacteria into typical and atypical strains and to evaluate the drug resistant pattern of mycobacterial isolates. The study aims to correlate multidrug resistance (MDR) and HIV seropositivity status in pulmonary tuberculosis patients. During the year 1994-1997, 750 pulmo...
متن کاملThe emerging crisis of drug-resistant tuberculosis in South Africa: lessons from New York City.
Multidrug-resistant tuberculosis and extensively drug-resistant tuberculosis have emerged as important infections in South Africa among patients infected with human immunodeficiency virus (HIV). In the face of this new epidemic, South Africa must rededicate itself to the task of tuberculosis control and treatment with a rapid, multifaceted approach. Priorities include expansion of second-line t...
متن کاملBlood cultures for the diagnosis of multidrug-resistant and extensively drug-resistant tuberculosis among HIV-infected patients from rural South Africa: a cross-sectional study
BACKGROUND The yield of mycobacterial blood cultures for multidrug-resistant (MDR) and extensively drug-resistant tuberculosis (XDR-TB) among drug-resistant TB suspects has not been described. METHODS We performed a retrospective, cross-sectional analysis to determine the yield of mycobacterial blood cultures for MDR-TB and XDR-TB among patients suspected of drug-resistant TB from rural South...
متن کاملEpidemic Levels of Drug Resistant Tuberculosis (MDR and XDR-TB) in a High HIV Prevalence Setting in Khayelitsha, South Africa
BACKGROUND Although multidrug-resistant tuberculosis (MDR-TB) is emerging as a significant threat to tuberculosis control in high HIV prevalence countries such as South Africa, limited data is available on the burden of drug resistant tuberculosis and any association with HIV in such settings. We conducted a community-based representative survey to assess the MDR-TB burden in Khayelitsha, an ur...
متن کاملEmergence of Increased Resistance and Extensively Drug-Resistant Tuberculosis Despite Treatment Adherence, South Africa
We investigated the emergence and evolution of drug-resistant tuberculosis (TB) in an HIV co-infected population at a South African gold mine with a well-functioning TB control program. Of 128 patients with drug-resistant TB diagnosed during January 2003-November 2005, a total of 77 had multidrug-resistant (MDR) TB, 26 had pre-extensively drug-resistant TB (XDR TB), and 5 had XDR TB. Genotyping...
