Acid-Base, Electrolyte and Fluid Alterations: Review

چکیده
Background: Intravenous fluids (IVF) are frequently utilized to restore intravascular volume in patients with distributive and hypovolemic shock. Although the benefits of the appropriate use of fluids in intensive care units (ICUs) and hospitals are well described, there is growing knowledge regarding the potential risks of volume overload and its impact on organ failure and mortality. To avoid volume overload and its associated complications, strategies to identify fluid responsiveness are developed and utilized more often among ICU patients. Apart from the amount of fluid utilized for resuscitation, the type of fluid used also impacts patient outcome. Colloids and crystalloids are two types of fluids that are utilized for resuscitation. The efficacy of each fluid type on the expansion of intravascular volume on one hand and the potential adverse effects of each individual fluid, on the other hand, need to be considered when choosing the type of fluid for resuscitation. The negative impact of hydroxyethyl starch on kidney function, of albumin on the mortality of head trauma patients and chloride-rich crystalloids on mortality and kidney function, are only examples of new develReceived: January 27, 2016 Accepted: April 19, 2016 Published online: May 18, 2016 Kianoush Kashani, MD Division of Nephrology and Hypertension Department of Medicine, Mayo Clinic 200 First Street SW, Rochester, MN 55905 (USA) E-Mail kashani.kianoush @ mayo.edu © 2016 S. Acid-Base, Electrolyte and Fluid Alterations: Review
Background: Potassium (K + ) is the major intracellular cation, with 98% of the total pool being located in the cells at a concentration of 140–150 mmol/l, and only 2% in the extracellular fluid, where it ranges between 3.5 and 5 mmol/l. A fine regulation of the intracellular-extracellular gradient is crucial for life, as it is the main determinant of membrane voltage; in fact, acute changes of...
متن کاملGeneral Therapeutic Considerations in Patients with Acute Renal Failure
PROLOGUE The purpose of this review is to provide the pharmacy student with an overview of the therapeutic considerations in patients with acute renal failure that has not responded to preliminary interventions (e.g., fluid therapy for restoration of plasma volume in patients with hypovolemia-induced renal dysfunction). A taxonomical listing of goals and objectives is provided to direct the stu...
متن کاملSelection of dialysate and replacement fluids and management of electrolyte and Acid-base disturbances.
Often, too little consideration is given to the fluids used in all forms of continuous renal replacement therapy (CRRT). However, errors in fluid prescription, delivery, or creation can be rapidly fatal; in addition, fluid associated expenses can be the overriding cost in continuous renal replacement therapies. While a standard solution is frequently acceptable in most clinical circumstances, s...
متن کاملDisorders of Acid-Base Balance: New Perspectives
Background: Disorders of acid-base involve the complex interplay of many organ systems including brain, lungs, kidney, and liver. Compensations for acid-base disturbances within the brain are more complete, while limitations of compensations are more apparent for most systemic disorders. However, some of the limitations on compensations are necessary to survival, in that preservation of oxygena...
