Postoperative fungal endophthalmitis caused by Trichosporon asahii treated with voriconazole.

چکیده
Postoperative fungal endophthalmitis is a rare but devastating complication of cataract surgery. Vitrectomy and intravitreal amphotericin B injection as well as administration of systemic antifungal agents have been suggested as optimal treatments for fungal endophthalmitis. However, this therapy may fail to eliminate fungal species resistant to current antifungal agents. The saprophytic fungus Trichosporon asahii is frequently observed as a cause of endogenous endophthalmitis in immunosuppressed patients. We report a case of postoperative endophthalmitis caused by T. asahii, resistant to amphotericin B. To the best of our knowledge, this is the first report of T. asahii endophthalmitis successfully treated with intravitreal and systemic voriconazole, pars plana vitrectomy, and removal of the intraocular lens and entire lens capsule.

Two Case Presentations Infected by Trichosporon asahii and Treated with Voriconazole Successfully
Background. Trichosporon asahii is an opportunistic fungus that causes infections in immunosuppressed patients. Neutropenia developing due to malignancies is an important risk factor for fungal infection. Case Report. We present two pediatric oncology cases successfully treated with voriconazole after T. asahii infection that is known to cause fatal sepsis and invasive fungal infection. Conclus...
متن کاملInvasive Trichosporon Infection: a Systematic Review on a Re-emerging Fungal Pathogen
Objectives: This review aimed to better depict the clinical features and address the issue of therapeutic management of Trichosporon deep-seated infections. Methods: We comprehensively reviewed the cases of invasive Trichosporon infection reported in the literature from 1994 (date of taxonomic modification) to 2015. Data from antifungal susceptibility testing (AST) studies were also analyzed. R...
متن کامل[Trichosporon asahii sepsis associated with urinary catheter in a pediatric burn unit: 2 case reports].
Trichosporon asahii is a ubiquitous fungus that has been isolated as part of human microbiota. There has been an emergence of this pathogen in recent years, causing superficial and deep seated infections. There are scarce reports of urinary tract infections in pediatric intensive care burn units caused by this agent. We describe the cases of 2 pediatric patients with prolonged hospitalization d...
متن کاملIn vitro activities of antifungal combinations against biofilms and planktonic forms of clinical Trichosporon asahii isolates.
Trichosporon species can cause biofilm-associated infections related to indwelling medical devices, especially intravenous catheters, and unacceptable mortality rates have been reported despite the administration of antifungal treatments (1). Trichosporon asahii can form biofilms with structured microbial communities in vitro, embedded within an extracellular matrix, with significantly increase...
متن کاملSuccessful treatment of septic shock with purpura fulminans caused by Trichosporon asahii in an immunocompetent patient.
Trichosporon asahii is an emerging mycosis characterized by high mortality rate in immunologically compromised patients. Only a few cases have been reported in immunocompetent subjects. We report a 46-yr-old man who had been healthy and who presented with septic shock and purpura fulminans caused by Trichosporon asahii. He responded well to antifungal therapy with amphotericin B and voriconazole.
