Upper Urothelial Carcinomas Associated with Balkan Endemic Nephropathy and Their Similarities with Upper Urothelial Carcinomas in Analgesic Nephropathy

چکیده
The incidence of upper urothelial carcinomas (UUC) is even more than hundred times higher in the foci of Balkan endemic nephropathy (BEN), by investigations of our team, and others. Every tenth patient suffers from both diseases. Etiology is unknown. It would seem that BEN is a tubulointerstitial nephritis associated with urothelial malignancy. We found that apoptosis plays an important role in the early phase of BEN, similarly as in other toxic nephropathies. Petronic shows great similarity between Balkan nephropathy and analgesic nephropathy, as well as between upper urothelial carcinomas associated with both diseases. Based on investigation of grade to stage ratio in 100 tumors from our series of UUC, the upper urothelial carcinomas from BEN areas are characterized by slow growth of tumor mass in comparison to the same carcinomas outside BEN regions in former Yugoslavia and the evaluated cut-off point % cErb-B2+ cells for aggressive tumors of grade 3 or stage 2 is slightly greater for the UUC in the general population. Smaller cell population in proliferation in the UUC from BEN regions than what is detected in the UUC outside these regions may explain BEN tumor resistance to putative toxic agent effect during tumor progression in grade 2, although the same toxin has probably realized tumor induction. It would seem that the cErb-B2 (a receptor for EGF) is differently involved in the control of a cell cycle proliferation in urothelial tumor associated with BEN.

Apoptosis and p53 status of the upper urothelial carcinomas from Balkan endemic nephropathy regions.
The upper urothelial carcinomas (UUC) associated with endemic nephropathy and analgesic nephropathy are considerably similar in many characteristics. These similarities diversify them from the same carcinomas in the general population w1x. Bilateral occurrence (simultaneous or successive) of the UUC is observed in about 8% cases of analgesic nephropathy and 10–14% in endemic Balkan nephropathy....

Balkan endemic nephropathy (BEN) is a chronic tubulointerstitial renal disease of a still unknown etiology, associated with an increased frequency of urothelial carcinoma, particularly of the upper urinary tract (UUT). The aim of the study was to compare the occurrence of UUT carcinomas between Brodsko-Posavska Region (BPR) which is the region with endemic villages and the non-endemic region of...

Endemic (Balkan) nephropathy is a chronic tubulointerstitial disease frequently accompanied by urothelial cell carcinomas of the upper urinary tract. This disorder has recently been linked to exposure to aristolochic acid, a powerful nephrotoxin and human carcinogen. Following metabolic activation, aristolochic acid reacts with genomic DNA to form aristolactam-DNA adducts that generate a unique...

An international symposium, held in Zagreb, Croatia, in October 2006, brought together basic scientists and clinical investigators engaged in research on endemic (Balkan) nephropathy, a chronic renal tubulointerstitial disease of previously unknown cause that often is accompanied by upper urinary tract urothelial cancer. Although this disease is endemic in rural areas of Bosnia, Bulgaria, Croat...
متن کاملPro- and Antiapoptotic Markers in Upper Tract Urothelial Carcinoma Associated with Balkan Endemic Nephropathy
The role of aristolochic acid in the etiology of Balkan endemic nephropathy (BEN) and associated upper-tract urothelial carcinoma (UTUC) has been recently confirmed. The aim of this study was to determine apoptosis-related marker(s) specific for BEN-associated UTUC. Present investigation included 105 patients with UTUC, 44 from BEN region and 61 control tumors. Altered expression of Survivin wa...
