چکیده

The segmentation of endovascular tools in fluoroscopy images can be accurately performed automatically or by minimum user intervention, using known modern techniques. It has been proven in literature, but no clinical implementation exists so far because the computational time requirements of such technology have not yet been met. A classical segmentation scheme is composed of edge enhancement filtering, line detection, and segmentation. A new method is presented that consists of a vector that propagates in the image to track an edge as it advances. The filtering is performed progressively in the projected path of the vector, whose orientation allows for oriented edge detection, and a minimal image area is globally filtered. Such an algorithm is rapidly computed and can be implemented in real-time applications. It was tested on medical fluoroscopy images from an endovascular cerebral intervention. Experiments showed that the 2D tracking was limited to guidewires without intersection crosspoints, while the 3D implementation was able to cope with such planar difficulties. Keywords—Edge detection, Line Enhancement, Segmentation, Fluoroscopy.

Electromagnetic tracking of flexible robotic catheters enables "assisted navigation" and brings automation to endovascular navigation in an in vitro study.

OBJECTIVE Combining three-dimensional (3D) catheter control with electromagnetic (EM) tracking-based navigation significantly reduced fluoroscopy time and improved robotic catheter movement quality in a previous in vitro pilot study. The aim of this study was to expound on previous results and to expand the value of EM tracking with a novel feature, assistednavigation, allowing automatic cathet...

Monoplane 3D Overlay Roadmap versus Conventional Biplane 2D Roadmap Technique for Neurointervenional Procedures

PURPOSE We investigated whether a 3D overlay roadmap using monoplane fluoroscopy offers advantages over a conventional 2D roadmap using biplane fluoroscopy during endovascular aneurysm treatment. MATERIALS AND METHODS A retrospective chart review was conducted for 131 consecutive cerebral aneurysm embolizations by three neurointerventionalists at a single institution. Allowing for a transitio...

Fully Automatic and Real-Time Catheter Segmentation in X-Ray Fluoroscopy

Augmenting X-ray imaging with 3D roadmap to improve guidance is a common strategy. Such approaches benefit from automated analysis of the X-ray images, such as the automatic detection and tracking of instruments. In this paper, we propose a real-time method to segment the catheter and guidewire in 2D X-ray fluoroscopic sequences. The method is based on deep convolutional neural networks. The ne...

Automatic Localization of Balloon Markers and Guidewire in Rotational Fluoroscopy with Application to 3D Stent Reconstruction

A fully automatic framework is proposed to identify consistent landmarks and wire structures in a rotational X-ray scan. In our application, we localize the balloon marker pair and the guidewire in between the marker pair on each projection angle from a rotational fluoroscopic sequence. We present an effective offline balloon marker tracking algorithm that leverages learning based detectors and...

Needle Detection and Tracking in the TIPS Endovascular Procedure

The TIPS procedure is an endovascular operation guided by biplane fluoroscopy. The goal is to pass a needle from the hepatic vein through the liver into the portal vein. We are developing a software for real-time, 3D guidance of the procedure. This paper discusses means of automatically defining the needle from biplane fluoroscopic views.
