Comment from Professor Andreas Schulze-Bonhage on: Lesion-focused stereotactic thermo-coagulation of focal cortical dysplasia II-B: A new approach to epilepsy surgery?

چکیده
An emerging strength of stereotactic RFTC is its high precision brain areas where conventional resection would carry a nificant risk for persistent neurological deficits. This observation s been made before by other groups who applied stereotactic TC to hypothalamic hamartomas and periventricular nodular terotopia. The volume-of-destruction achieved by RFTC can be anticipated liably. Commercially available brachytherapy software allows e planning of complex three-dimensional ablations. The elective uation of preoperative planning facilitates the integrating of ultimodal imaging information. During surgery, the validity of esurgical assumptions regarding the epileptogenicity of the sion and the proximity or distance to eloquent structures can be termined via the very electrode that is subsequently used for agulation. This feedback is an advantage of RFTC over other structive surgical techniques such as stereotactic radiosurry. Another advantage over radiosurgery is that the lesions are mediately destroyed without significant oedema.

Comment from Professor Dirk Van Roost on: Lesion-focused stereotactic thermo-coagulation of focal cortical dysplasia II-B: A new approach to epilepsy surgery?
The idea of attempting to eliminate epileptogenic foci by stereotactic means is not novel; it has been pursued since the 1960s. A range of ablation techniques has been used: from simple electrocoagulation to thermo-coupled electrocoagulation or radiosurgery, and, most recently, focused ultrasound. If the results of stereotactic treatment for epilepsy surgery have not been completely convincing ...
متن کاملLesion focused stereotactic thermo-coagulation of focal cortical dysplasia IIB: A new approach to epilepsy surgery?
PURPOSE Over the last few decades, preferred epilepsy surgical approaches have developed from standard lobectomies to individually tailored resections. Yet, it remains uncertain how small surgical interventions can be without compromising the success of the surgery. Particularly in patients with epileptogenic lesions identified by magnetic resonance imaging (MRI), it is unresolved whether resec...
متن کاملMulti-focal occurrence of cortical dysplasia in epilepsy patients.
This study describes the existence and the clinical and electrophysiological features of multi-focal cortical dysplasia in epilepsy patients. Five patients with intractable focal epilepsy are reported. All patients underwent invasive presurgical video-electroencephalography monitoring. Localization of dysplastic areas was based on high-resolution magnetic resonance scanning, surface and intracr...
متن کاملLesion guided radiofrequency thermocoagulation (L-RFTC) for hypothalamic hamartomas, nodular heterotopias and cortical dysplasias: Review and perspective
Lesion guided radiofrequency thermocoagulation (L-RFTC) via stereotactically inserted coagulation probes is a further development of stereotactic thermocoagulation thalamotomy and stereo-EEG guided RFTC. In this method epileptogenic lesions detected via magnetic resonance imaging (MRI) move to the center of coagulation planning. Two surgical strategies can be applied: lesion disconnection and l...
متن کاملAssessment of cognitive functions before and after stereotactic interstitial radiosurgery of hypothalamic hamartomas in patients with gelastic seizures.
We assessed cognitive functions before and 3 months after interstitial radiotherapy in 14 patients with gelastic seizures caused by hypothalamic hamartoma. Cognitive functioning was assessed before temporary implantation of (125)I-seed and 3 months after seed explantation. Performance was compared with that of a selected control group of conservatively treated patients with symptomatic focal ep...
