چکیده

OBJECTIVES The cingulum and fornix are important structures for memory function. Using follow-up diffusion tensor tractography in a patient with traumatic brain injury we found degeneration of the cingulum and fornix. CASE REPORT An 18-year-old male who had had a traffic accident underwent conservative management for diffuse axonal injury. Brain magnetic resonance imaging showed an encephalomalactic lesion in the posterior portion of the corpus callosum. The patient had severe cognitive problems at 3 months after onset. However, while his intelligence had improved, his memory impairment had been aggravated at 14 months from onset. RESULTS On the first diffusion tensor tractographies, the integrity of both corticospinal tracts, right cingulum, and left fornix were preserved; however, compared with controls, there were disruptions in both ends of the left cingulum and right fornical crus. On the second diffusion tensor tractographies, both the cingulum and fornix showed severe degeneration, although the integrities of both corticospinal tracts were well preserved. CONCLUSION We conclude that patients with memory impairment following traumatic brain injury should be evaluated using diffusion tensor imaging. In addition, follow-up diffusion tensor imaging may be necessary in patients with sustained memory impairment.

Comparison of Limbic Regions FA Using Tractography-defined ROIs in AD and MCI

Introduction: The fornix and cingulum (limbic regions) are suspected of being affected by mild cognitive impairment (MCI) and Alzheimer Disease (AD). Recent studies have reported observable reductions in FA of the cingulum between normal and AD populations. Most studies resort to manual tracing of the anatomy in patient acquisition space. Objective isolation of the fornix and cingulum has remai...

Diffusion tensor tractography of traumatic diffuse axonal injury.

BACKGROUND Diffuse axonal injury is a common consequence of traumatic brain injury that frequently involves the parasagittal white matter, corpus callosum, and brainstem. OBJECTIVE To examine the potential of diffusion tensor tractography in detecting diffuse axonal injury at the acute stage of injury and predicting long-term functional outcome. DESIGN Tract-derived fiber variables were ana...

Recovery of an injured cingulum via an aberrant neural tract in a patient with traumatic brain injury

BACKGROUND We report on a patient who appeared to show recovery of an injured anterior cingulum via an aberrant neural tract between an injured cingulum and the basalis nucleus of Meynert following traumatic brain injury (TBI), which was demonstrated on diffusion tensor tractography (DTT). METHODS A 47-year-old male who had suffered a pedestrian traffic accident underwent conservative managem...

Longitudinal changes of structural connectivity in traumatic axonal injury.

OBJECTIVES To identify structural connectivity change occurring during the first 6 months after traumatic brain injury and to evaluate the utility of diffusion tensor tractography for predicting long-term outcome. METHODS The participants were 28 patients with mild to severe traumatic axonal injury and 20 age- and sex-matched healthy control subjects. Neuroimaging was obtained 0-9 days postin...
