چکیده

Operative procedure The pedicle screws in the proximal site of osteotomy are not connected to the rod to permit spinal shortening in the postoperative course. When a connector is used between a pedicle screw and a rod, the connector is not fixed to the rod to allow it to slide on the rod. When a connector is not used, the rod is only placed in the screw head with reinforcement by some sets of cranial sublaminar taping. U-rod is used to not injure the skin when postoperative junctional kyphosis (PJK) occurs.

Surgical Strategies to Improve Fixation in the Osteoporotic Spine: The Effects of Tapping, Cement Augmentation, and Screw Trajectory

Study Design Biomechanical study of pedicle screw fixation in osteoporotic bone. Objective To investigate whether it is better to tap or not tap osteoporotic bone prior to placing a cement-augmented pedicle screw. Methods Initially, we evaluated load to failure of screws placed in cancellous bone blocks with or without prior tapping as well as after varying the depths of tapping prior to screw ...

Pullout performance comparison of pedicle screws based on cement application and design parameters.

Pedicle screws are the main fixation devices for certain surgeries. Pedicle screw loosening is a common problem especially for osteoporotic incidents. Cannulated screws with cement augmentation are widely used for that kind of cases. Dual lead dual cored pedicle screw has already given promising pullout values without augmentation. This study concentrates on the usage of dual lead dual core wit...

Surgical treatment of the osteoporotic spine with bone cement-injectable cannulated pedicle screw fixation: technical description and preliminary application in 43 patients

OBJECTIVES To describe a new approach for the application of polymethylmethacrylate augmentation of bone cement-injectable cannulated pedicle screws. METHODS Between June 2010 and February 2013, 43 patients with degenerative spinal disease and osteoporosis (T-score <-2.5) underwent lumbar fusion using cement-injectable cannulated pedicle screws. Clinical outcomes were evaluated using a Visual...

PMMA-augmentation of incompletely cannulated pedicle screws: a cadaver study to determine the benefits in the osteoporotic spine.

BACKGROUND Pedicle screw pullout due to poor bone quality, mainly caused by osteoporosis, is a common problem in spine surgery. Special implants and techniques, especially PMMA augmentation, were developed to improve the fixation of pedicle screws. PMMA injection into a pilot hole or through a screw involves the same risks as vertebroplasty or kyphoplasty, regardless of the technique used. Espe...

Asymptomatic Bone Cement Leakage into Inferior Vena Cava during Cement Augmentation of Pedicle Screw for Osteoporotic Vertebral Collapse

Bone cement augmentation of pedicle screw is an important surgical procedure for osteoporotic vertebral collapse, preventing screw loosening, migration or back-out. Bone cement leakage may develop and mostly does not compress the peripheral tissue of the vertebral body including the spinal cord. However, to our best knowledge, bone cement leakage into the inferior vena cava during cement augmen...
