Monoamniotic Twins: A Case Series
نویسندگان
چکیده
Monoamniotic twinning is a rare event associated with a high perinatal morbidity and mortality. In this article, six pairs of monoamniotic twins diagnosed prenatally during 2001-2003 were evaluated. The management protocol included documentation of chorionicity and amnionicity by early ultrasonographic examination, 2-4 weekly serial ultrasound surveillance, antepartum fetal heart rate monitoring initiated at 28 weeks of gestation and a non-interventional approach beyond 32 weeks. In 5 cases, the detection of monoamnionicity was made during the first trimester. Cord entanglement was found in four cases. In only one case (acardiac twin) was there discordance for structural abnormality. In all cases, the pregnancy continued beyond 20 weeks, and with a non-interventional management policy, a live birth rate of 80% beyond 24 weeks was achieved. We believe that, at any given gestation, if prematurity complications are likely to outweigh the risk of an uncertain IUFD, a non-interventional management may be a reasonable option.

Case Series of Monoamniotic and Pseudomonoamniotic Twin Gestations
We retrospectively evaluated a series of 18 monoamniotic and 7 pseudomonoamniotic (secondary to rupture in the membrane dividing monochorionic diamniotic twins) twin gestations managed after 20 weeks' gestation. There were no significant differences in the incidence of neonatal death or umbilical cord entanglement between the monoamniotic and pseudomonoamniotic twin gestations (33 versus 21%, P...
متن کاملInfantile Perineal Protrusion in Two Monochorionic Twins
Case Report Two female monochorionic-monoamniotic twins showed the same kind of infantile perineal protrusion (IPP) at birth. Lesions in both twins progressively healed until resolution in 6 weeks' time; none of the twins have manifested, till date, alvus disturbances. Discussion and Literature Review A literature review numbers approximately 100 reports of IPP. This condition has been classica...
متن کاملMonoamniotic Twins with One Fetal Anencephaly and Cord Entanglement Diagnosed with Three Dimensional Ultrasound at 14 Weeks of Gestation
A 29-year-old pregnant woman with parity 0-0-0-0 was diagnosed with monoamniotic twin pregnancy discordant for anencephaly at 14 weeks gestation. Umbilical cord entanglement, which is an important cause of fetal death in monoamniotic twins, was confirmed by three-dimensional ultrasound. Cesarean section was performed at 34 weeks of gestation, and the normal newborn infant was discharged without...
متن کاملSelective feticide in monoamniotic twin pregnancies by umbilical cord occlusion and transection.
OBJECTIVES In monoamniotic twin pregnancies discordant for fetal anomaly, parents may opt for selective feticide. However, the normal co-twin remains at risk of sudden demise from cord entanglement. We report on three cases of successful selective feticide by cord occlusion combined with cord transection. METHODS We describe technical details and outcome of three monoamniotic twin pregnancies...
متن کاملCloaca in Discordant Monoamniotic Twins: Prenatal Diagnosis and Consequence for Fetal Lung Development
Objective Describe a case of cloaca prenatally diagnosed in one of a set of monoamniotic twins. Study Design Retrospective review of a case. Results Cloaca is one of the most complex and severe degrees of anorectal malformations in girls. We present a discordant cloaca in monoamniotic twins. Fetal ultrasound showed a female fetus with a pelvic midline cystic mass, a phallus-like structure, a pr...
