Comparative biomechanical evaluation a lumbar spacer with integrated screws

چکیده

INTRODUCTION The goal of lumbar spinal fusion is to limit pain by decompression and removal of soft tissues, followed by the formation of a rigid bony union between adjacent vertebrae. The general consensus for a rigid fusion construct requires careful preparation of the intervertebral space and the creation of a rigid fusion construct. Anterior lumbar interbody fusion (ALIF) provides good access to the intervertebral space and has been shown to yield excellent fusion results, especially with supplemental posterior fixation. However, supplemental posterior fixation requires a secondary procedure and is associated with additional complications. Threaded interbody cages provide one stand-alone alternative, but have issues including imaging limitations and subsidence risks. A PEEK interbody cage has been developed with incorporated screws so that additional fixation and resistance to graft extrusion could be provided without the use of blocking screws, plates, or pedicular fixation.

Biomechanical Characteristics of an Integrated Lumbar Interbody Fusion Device

INTRODUCTION We hypothesized that an Integrated Lumbar Interbody Fusion Device (PILLAR SA, Orthofix, Lewisville, TX) will function biomechanically similar to a traditional anterior interbody spacer (PILLAR AL, Orthofix, Lewisville, TX) plus posterior instrumentation (FIREBIRD, Orthofix, Lewisville, TX). Purpose of this study was to determine if an Integrated Interbody Fusion Device (PILLAR SA) ...

Biomechanical Evaluation of a Standalone Lumbar Interbody Cage with Integrated Screws

INTRODUCTION: Spinal fusion techniques including anterior lumbar interbody fusion (ALIF) have been used to alleviate chronic low back pain and segmental instability. Supplemental fixation using pedicle screws, translaminar screws, facet screws, anterior plates or spinous process plates is often used to increase the stability of the ALIF construct to enhance fusion and prevent subsidence and mig...

The Change of Sagittal Alignment of the Lumbar Spine after Dynesys Stabilization and Proposal of a Refinement

OBJECTIVE Dynesys® is one of the pedicle-based dynamic lumbar stabilization systems and good clinical outcome has been reported. However, the cylindrical spacer between the heads of the screws undergoes deformation during assembly of the system. The pre-strain probably change the angle of instrumented spine with time and oblique-shaped spacer may reduce the pre-strain. We analyzed patients with...

Biomechanics of Lateral Interbody Spacers: Going Wider for Going Stiffer

This study investigates the biomechanical stability of a large interbody spacer inserted by a lateral approach and compares the biomechanical differences with the more conventional transforaminal interbody fusion (TLIF), with and without supplemental pedicle screw (PS) fixation. Twenty-four L2-L3 functional spinal units (FSUs) were tested with three interbody cage options: (i) 18 mm XLIF cage, ...
