Foetal heart rate monitoring--present lessons and future developments.

چکیده
JUST as the first detailed description of the foetal heart sounds (Le Jumeau de Kergaradec, 1822) eventually led to the view that foetal bradycardia might indicate danger to the foetus (Von Winckel, 1903) and to the present routine practice of periodic auscultation during labour, so the first reported foetal electrocardiogram (Cremer, 1906) has been followed by efforts to develop automatic methods of monitoring the foetal heart (Hon, 1960). The shortcomings of intermittent auscultation as a means of detecting foetal distress or of predicting the condition of the baby at and after delivery prompt the following questions: Is electronic monitoring any better ? Is it now ready for routine use in our labour wards Has research in this field provided any lessons which can be applied now to routine practice ? How are future advances likely to help clinical management ? In attempting to make some answer to these questions, the writer draws heavily on recent experience as a member of Dr. Edward H. Hon's research group in Los Angeles and at Yale University. In addition to obtaining practical training in Hon's methods, his unrivalled library of tracings from more than 1,000 fully monitored labours was studied in detail.

Electronic foetal heart-rate monitoring In current practice, electronic foetal heart-rate monitoring is performed by either the internal or the external method. With the internal method, an ECG electrode is applied directly to the foetus, and the monitor separates the foetal electrocardiogram from the maternal, amplifies the foetal signal and measures the intervals between consecutive foetal he...
متن کاملComparative Study of Foetal Colour Doppler versus Non-Stress Test as a Predictor of Perinatal Outcome in High Risk Pregnancy
The desire of every women contemplating motherhood is that her pregnancy culminates in a healthy offspring who will achieve the highest possible physical and mental potential. Assessment of the foetal wellbeing is done by various biophysical methods. But the problem of sample collection, accuracy and need for laboratory technology and personnel have made biochemical methods of foetal monitoring...
متن کاملA practical guide to non-invasive foetal electrocardiogram extraction and analysis.
Non-Invasive foetal electrocardiography (NI-FECG) represents an alternative foetal monitoring technique to traditional Doppler ultrasound approaches, that is non-invasive and has the potential to provide additional clinical information. However, despite the significant advances in the field of adult ECG signal processing over the past decades, the analysis of NI-FECG remains challenging and lar...
متن کاملThe monitoring of bovine pregnancies derived from transfer of in vitro produced embryos.
Both an increased rate of embryonic, foetal and perinatal losses, and the occurrence of deviations in foetal and placental development are associated with bovine pregnancies obtained from in vitro produced embryos. This thus requires for a more accurate and frequent monitoring of foetal and maternal functions during pregnancies. Such approaches will enable to establish the period during which t...
متن کاملI-20: ART - Children How Are They Doing Lessons from Research
Worldwide there are over 4,000,000 ART conceived individuals. As fertility rates ‘fall’ in some countries use of ART is increasing with rates of 1.8% of live births in the UK, 4.4% in Denmark etc. Although still the single largest threat to future ART born children is being born twin, triplet or more, other research shows that there is good grounds for monitoring of the health of these children...
