Induction of supraventricular tachycardia by ventricular pacing: what is the mechanism?
نویسنده
چکیده
A 29-year-old woman underwent an electrophysiologic procedure because of a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia. The baseline cycle length, atrial-His (AH), and His-ventricular (HV) intervals were 550, 50, and 45 msec, respectively. The AV block cycle length was 270 msec. With programmed atrial stimulation, there was no evidence of dual AV nodal pathways or inducible tachycardia. During ventricular pacing, there was occasional VA conduction at cycle lengths of 500 to 400 msec, and VA dissociation at paced cycle lengths below 400 msec (Fig. 1). On several occasions upon cessation of ventricular pacing, a supraventricular tachycardia with a cycle length of 300 msec was initiated (Fig. 2). What is the mechanism of the tachycardia, and how was it initiated?

Atrial pacing during a narrow QRS complex tachycardia: what is the mechanism?
A 25-year-old woman with a 5-year history of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length, atrial–His-bundle (AH) interval, and His-bundle–ventricular (HV) interval were 750, 60, and 50 msec, respectively. Paroxysmal supraventricular tachycardia with a cycle length of 320 msec was induced by programmed atrial stimulation with...

Case Presentation A 34-year-old man with a 10-year history of paroxysmal supraventricular tachycardia underwent an electiophysiologic procedure. A narrowQRS complex tachycardia with a cycle length of 260 to 280 msec reproducibly was induced by right atrial overdrive pacing. At times there was a 1:1 relationship between the atrium and ventricle (Fig. 1), and at other times there was Mobitz I sec...

A 66-year-old woman with a 24-year history of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length, atrial-His (AH), and Hisventricular (HV) intervals were 8(X). 70, and 40 msec, respectively. Programmed stimulation with a single atrial extrastimulus did not demonstrate dual AV nodal physiology and did not result in the induction of ...
متن کاملThe response of paroxysmal supraventricular tachycardia to overdrive atrial and ventricular pacing: can it help determine the tachycardia mechanism?
INTRODUCTION Standard electrophysiologic techniques generally allow discrimination among mechanisms of paroxysmal supraventricular tachycardia. The purpose of this study was to determine whether the response of paroxysmal supraventricular tachycardia to atrial and ventricular overdrive pacing can help determine the tachycardia mechanism. METHODS AND RESULTS Fifty-three patients with paroxysma...
متن کاملRight ventricular entrainment of a narrow QRS complex tachycardia: what is the mechanism?
A 65-year-old woman underwent an electrophysiologic procedure because of a 20-year history of paroxysmal supraventricular tachycardia. The baseline sinus cycle length was 800 msec, with an atrial-His interval (AH) of 100 msec and a His-ventricular interval (HV) of 40 msec. There was evidence of dual AV nodal pathways, but no inducible tachycardia. During infusion of isoproterenol, the sinus cyc...
