چکیده

BACKGROUND The minimum time interval between administration of oral midazolam and separation of children from their parents that ensures good anterograde amnesia has not been previously determined. This is of particular importance in a busy operating room setting where schedule delays secondary to midazolam administration may not be tolerated. METHODS Children (n = 113) undergoing general anesthesia and surgery completed preoperative baseline memory testing using a validated series of picture cards and were randomly assigned to one of three midazolam groups or a control group. Exactly, 5, 10, or 20 min after receiving oral midazolam (0.5 mg/kg) or 15 min after receiving placebo, children were administered a second memory test that used pictures. Anxiety of children was assessed during induction of anesthesia with use of a validated anxiety measurement tool. Postoperatively, recall and recognition for picture cards seen during baseline testing and postintervention testing were assessed. RESULTS Postoperatively, recall and recognition of pictures presented to patients after drug administration (anterograde amnesia) showed significant group differences (P = 0.0001), with recall impaired in the 10- (P = 0.004) and 20-min groups (P = 0.0001). Similarly, recognition memory was impaired in the 5- (P = 0.0008), 10- (P = 0.0001) and 20-min (P = 0.0001) groups. Significant anxiolytic effects of midazolam were observed as early as 15 +/- 4 min after midazolam administration (P = 0.02). CONCLUSIONS Midazolam administered orally produces significant anterograde amnesia when given as early as 10 min before a surgical procedure.

Oral midazolam for conscious sedation of children during minor procedures.

OBJECTIVE To compare the safety and efficacy of two doses of oral midazolam, and to assess the drug induced amnesia obtained, when used for conscious sedation of children undergoing minor procedures in the accident and emergency (A&E) setting. METHODS A two stage trial was completed: an initial prospective, double blinded, randomised trial comparing 0.2 mg/kg midazolam suspension with 0.5 mg/...

Comparing the effect of sedation with two different ratios of Ketofol on amnesia in children with acute leukemia who are candidate for intrathecal injection of chemotherapy drugs

Background: Ketamine increases blood pressure and heart rate. Propofol is an anesthetic drug with rapid recovery, but it causes respiratory depression, low heart rate and low blood pressure. Combination of ketamine and propofol provides sedation, analgesia, and rapid recovery with hemodynamic stability and minimal respiratory depression. There have been a few studies about the effect of differe...

A Comparative Study between Intramuscular Midazolam and Oral Clonidine as a Premedication for General Anesthesia

Background: Most anesthesiologists agree on the need for efficient pre-medication. The pattern of desired effects of a pre-medication is however, complex and includes relief of anxiety, sedation and relaxation of the patient. The present study was undertaken to compare the effects of Midazolam and clonidine as premedication. Methodology: A comparative study between midazolam and clonidine as a ...

Anxiety in Children Undergoing VCUG: Sedation or No Sedation?

BACKGROUND Voiding cystourethrograms are distressing for children and parents. Nonpharmacological methods reduce distress. Pharmacological interventions for VCUG focus on sedation as well as analgesia, anxiolysis, and amnesia. Sedation has cost, time, and safety issues. Which agents and route should we use? Are we sure that sedation does not influence the ability to diagnose vesicoureteric refl...

Paediatric Premedication: A Comparison of Sublingual Buprenorphine and Midazolam in Children (4-10 Years) Scheduled for Adenotonsillectomyt

Introduction and Objectives. Preanesthetic medication may reduce the risks of adverse psychological and physiological sequela of induction of anesthesia in children. Administration of premedication by sublingual route may provide the best compromise, that is, relatively rapid absorption without causing pain. In this study, we compared sedative and anxiolytic effects of midazolam and buprenorphi...
