چکیده
Issue 1: Arthroplasty vs ORIF • Non-displaced or minimally displaced fractures-standard treatment is closed reduction and internal fixation in all ages. o Hudson, CORR, 1998. At 8 year f/u Nondisplaced treated with ORIF-no difference in revision rate compared to arthroplasty Displaced treated with ORIF – higher 8 year cumulative revision rate in older patients compared to arthroplasty o However, patients with previous symptomatic hip arthritis or those with pathologic fractures may benefit from primary THA. • Displaced femoral neck fractures in the elderly – Arthroplasty is preferred. • Argument for ORIF rather than arthroplasty: o retaining a patient's femoral head will result in better hip function o quicker o cheaper o less blood loss o lower dislocation rate • Argument for THA rather than ORIF : o Faster mobilization, immediate WBAT o Less pain o Lower revision rate the rate of reoperation following internal fixation of femoral neck fractures is relatively high. A meta-analysis (Bhandari, JBJS, 2003)-rates of osteonecrosis and nonunion in hip fractures treated with fixation as high as 20% and 30%, respectively. • The relative risk of revision surgery after arthroplasty was LOWER compared with the risk after internal fixation (RR=0.23, p = 0.0003).

Bilateral Simultaneous Femoral Neck and Shafts Fractures – A Case Report
Simultaneous fractures of the femoral neck and shaft are not common injuries, though they cannot be considered rare. Herein, we report our experience with a patient with bilateral occurance of this injury. Up to the best of our knowkedge this is the first case reported in literature in which correct diagnosis was made initially. Both femurs were fixed using broad 4.5 mm dynamic compression pl...
متن کاملThe impact of high total cholesterol and high low-density lipoprotein on avascular necrosis of the femoral head in low-energy femoral neck fractures
BACKGROUND Avascular necrosis of the femoral head (AVNFH) typically constitutes 5 to 15% of all complications of low-energy femoral neck fractures, and due to an increasingly ageing population and a rising prevalence of femoral neck fractures, the number of patients who develop AVNFH is increasing. However, there is no consensus regarding the relationship between blood lipid abnormalities and p...
متن کاملAssessment of Quality of Life among Elderly Patients with Femoral Neck Fractures Surgically Treated by Partial Hip Arthroplasty
INTRODUCTION Medical science, in the past few years, has presented a convergence towards studies addressing quality of life. With time, some typical diseases of age groups emerge and, among the ones most affecting elderly individuals, the femoral neck fracture can be mentioned(1). The mean age for femoral neck fractures is 75-80 years for women, being a little lower among men: 70-75 years. Surg...
متن کاملAn atypical femoral neck fracture: a management dilemma
Femoral neck fractures are common in the elderly and are broadly grouped into either intracapsular or extracapsular fractures. We report an unusual femoral neck fracture that had features of both and discuss the management of such a case.
متن کاملScreening for osteomalacia in elderly patients with femoral neck fractures.
In this study 201 elderly patients with femoral neck fractures were compared with 30 osteomalacic patients with the same injury. Hypocalcaemia and a raised alkaline phosphatase level are common biochemical abnormalities in elderly patients with femoral neck fractures. In only a minority of patients, however, were they associated with histologically proven osteomalacia. By using the combination ...
متن کاملNon-operative treatment of elderly patients with femoral neck fracture.
Surgery is the routine management for elderly individuals with femoral neck fracture, in order to reduce the morbidity and the mortality and to relieve pain. Sixteen elderly patients with displaced femoral neck fractures who were unfit for surgeries were treated conservatively. The mortality in this group was not higher than reported following surgical management. Close nursing care, physiother...
